超声引导下插管溶栓治疗急性下肢深静脉血栓的效果分析

Effect Analysis of Ultrasound-Guided Intubation Thrombolysis in Treatment of Acute Lower Extremity Deep Vein Thrombosis

目的:探讨超声引导下插管溶栓治疗急性下肢深静脉血栓(DVT)的效果。方法:选取2020年6月—2022年6月于江苏大学附属武进医院诊治的86例DVT患者作为研究对象,按照随机数字表法分为超声插管组与全身溶栓组,各43例。两组基础治疗均相同,超声插管组予以超声引导下插管溶栓,全身溶栓组予以足背浅静脉全身溶栓,比较两组各项评价指标水平、术后不同时间Villalta评分。结果:超声插管组尿激酶用量低于全身溶栓组,住院时间短于全身溶栓组,肢体消肿率、血栓溶解率高于全身溶栓组,差异有统计学意义(P=0.000)。超声插管组术后3、6、9、12个月Villalta评分均低于全身溶栓组,差异有统计学意义(P<0.05)。结论:超声引导下插管溶栓可以提高急性下肢深静脉血栓的疗效,减少药物用量,对深静脉远期恢复有益。

Objective:To investigate the effect of ultrasound-guided intubation thrombolysis in the treatment of acute lower extremity deep vein thrombosis(DVT).Methods:From June 2020 to June 2022,86 patients with DVT admitted to Wujin Hospital Affiliated to Jiangsu University were selected as the study subjects.They were divided into ultrasound intubation group and systemic thrombolysis group according to random number table method,with 43 cases in each group.The basic treatment in two groups was the same.The ultrasound intubation group was given ultrasound-guided intubation thrombolysis.The systemic thrombolysis group was given systemic thrombolysis in the superficial dorsal vein of the foot.Each evaluation index and Villalta score at different time point after surgery were compared between the two groups.Results:The dosage of urokinase in the ultrasound intubation group was lower than that in the systemic thrombolysis group,the length of hospital stay in the ultrasound intubation group was shorter than that in the systemic thrombolysis group,the limb detumescence rate and thrombolysis rate in the ultrasound intubation group were higher than those in the systemic thrombolysis group,and the difference was statistically significant(P=0.000).At 3,6,9,12 months after surgery,Villalta score in the ultrasound intubation group was lower than that in the systemic thrombolysis group,and the difference was statistically significant(P<0.05).Conclusion:Ultrasound-guided intubation thrombolysis can improve the efficacy of acute lower extremity deep vein thrombosis,reduce drug dose and is beneficial for the long-term recovery of deep vein.
