探讨情景模拟在围术期血管活性药物规范应用中的培训效果

摘要 目的 探讨情景模拟在围术期血管活性药物规范应用中的培训效果。方法 选择2020级和2021级麻醉住院医师各5名,随机均分为情景模拟教学组(观察组)和传统教学组(对照组)。观察组先多媒体授课,再模拟教学并全程录像,然后回放录像进行讨论;对照组采用多媒体授课,课后布置内容复习。课程结束后两周,两组住院医师进行理论和技能考核。结果 在情景模拟技能考核中,观察组与对照组在药物选择上成绩分别为(9.60±0.25)分比(7.40±0.87)分,差异有统计学意义(P<0.05);而在疾病知识、循环管理、气道管理、药物剂量、用药时机、患者转归和团队协作方面,两组比较差异无统计学意义(P> 0.05)。观察组住院医师认为模拟教学符合临床实践、能考查临床水平、提高临床思维能力及团队合作意识。结论 情景模拟教学可提高住院医师理论、技能成绩,更易掌握血管活性药物的应用。 Objective To explore the learning effect of scenario simulation in the standardized application of vasoactive drugs during perioperative period.Methods Ten students from the standardized training of anesthesia residents in 2020 and 2021 were randomly divided into the situational simulation teaching group(observation group)and the traditional teaching group(control group).In observation group,theory teaching was applied,videotaped the whole course in the simulated operating room,and then the residents discussed their performance by the playback of the simulated video.In control group,a multimedia courseware was applied firstly and then post-class materials were conducted.The examinations of theory and skill were assessed in two groups after two weeks.Results For skill assessment of scenario simulation,the scores of observation group and control group in drug selection were(9.60±0.25)versus(7.40±0.87)with significant difference(P<0.05);there was no significant difference between the two groups in disease knowledge,circulation management,airway management,drug dose,medication timing,patient outcome and teamwork(P>0.05).In observation group,all of the residents thought that the simulation teaching conformed to clinical practice and could really exam the clinical ability,improve the ability of clinical thinking and team cooperation consciousness. In addition, 40 % of the residents believed that the simulation teaching waslack of realistic and communication ability with patients. While 20 % of the residents think that the teaching hour isgenerally reasonable. Conclusion Situational simulation teaching contribute to residents' theory and skill learning,and easily facilitate the master of vasoactive drugs.
