三级甲等医院家庭医生签约服务运行现状及续约相关影响因素的研究

Study on the Operation of Family Signing Service in Grade-A Tertiary Hospital

目的通过分析四川大学华西医院全科医学科开展的家庭医生签约服务相关数据,讨论影响签约服务质量的相关因素,探索其发展方向。方法选取2018年7月-2020年9月在四川大学华西医院全科医学科办理过“健康人生”家庭庭医生签约服务的人员,统计基本信息登记表,分析探索影响续约的因素。结果研究期限内登记在册的签约人员共1818人,以汉族男性为主,平均签约年龄为(57.74±14.16)岁,签约人员大专及以上学历占比为61.28%,整体呈现出以成都市武侯区为中心向周围区域逐步扩散的分布态势。年龄、是否有不良生活习惯以及是否有基础疾病是影响续约的因素(P<0.05)。结论目前签约人群表现出鲜明的群体特征,需要对当前签约中占比较少的女性、大专以下学历、少数民族等人群予以更多关注,以具体需求为导向优化服务内容;续约情况确实会受一些因素的影响,需要进一步挖掘数据深度,提升续约率;在三甲医院开展签约服务具有较广泛的社会需求,需要扩大服务范围,持续改进服务模式。

Objective By analyzing the relevant data of family doctor signing service carried out by the general medicine department of West China Hospital of Sichuan University,this paper discusses the relevant reasons affec-ting the quality of signing service,and explores the development direction of family signing service carried out by Grade-A tertiary hospital.Methods Personnel who had handled the"healthy life"family signing service in the ge-neral medicine department of West China Hospital of Sichuan University from July 2018 to September 2020 were selected.Data were collected form the basic information registration form and analyzed by descriptive statistical analysis.Through descriptive analysis,t-test,χ^(2) test,logistic regression and other methods were used to explore the factors affecting contract renewal.Results During the study period,a total of 1818 contractors were registered,mainly Han males,with an average age of(57.74±14.16)years.The proportion of contractors with college degree or above accounted for 61.28%.As a whole,they gradually spread from Wuhou District of Chengdu to the surrounding areas.Age,bad living habits and basic diseases were the factors that affect the renewal of the contract(P<0.05).Conclusion At present,the signing population shows distinct group characteristics,and more attention needs to be paid to women,people with college degrees or below,ethnic minorities and other people who currently sign contracts,so as to optimize the service content based on specific needs.The renewal situation will indeed be affected by some factors,so we need to further mine the depth of data and improve the renewal rate.There is a wide range of social needs to carry out contracted services in class III A hospitals,so it is necessary to expand the scope of services and conti-nuously improve the service mode.
