2型糖尿病病人皮肤瘙痒危险因素的荟萃分析

Risk factors for pruritus in patients with type 2 diabetes mellitus:a Meta-analysis

目的:探讨2型糖尿病病人皮肤瘙痒的危险因素。方法:检索中国知网(CNKI)、万方数据库(WanFang Database)、维普数据库(VIP Database)、中国生物医学文献数据库(CBM)、PubMed、EMbase、Web of Science和the Cochrane Library数据库,筛选建库至2022年6月13日关于2型糖尿病病人皮肤瘙痒的病例对照研究,进行质量评价及资料提取后,采用RevMan 5.3和Stata12.0软件进行荟萃分析。结果:共纳入14篇文献,涉及1381例病人。2型糖尿病病人皮肤瘙痒的主要危险因素为:年龄[OR=1.28,95%CI(1.19,1.38),P<0.001]、病程[OR=1.13,95%CI(1.06,1.21),P=0.003]、糖化血红蛋白[OR=1.71,95%CI(1.33,2.19),P<0.001]、空腹血糖[OR=1.55,95%CI(1.21,1.99),P<0.001]、血脂异常[OR=2.56,95%CI(1.99,3.29),P<0.001]、糖尿病性视网膜病变[OR=1.14,95%CI(1.07,1.21),P<0.001]、糖尿病周围神经病变[OR=1.37,95%CI(1.03,1.81),P=0.030]。结论:高龄、长病程、高糖化血红蛋白值、高空腹血糖值、血脂异常、合并糖尿病性视网膜病变、合并糖尿病周围神经病变是2型糖尿病病人皮肤瘙痒的主要危险因素,而有糖尿病肾病、吸烟对皮肤瘙痒的致病作用尚不确定。

Objective:To investigate the risk factors of pruritus in patients with type 2 diabetes mellitus(T2DM).Methods:The case-control studies on pruritus in T2DM patients were retrieved from China National Knowledge Infrastructure(CNKI),WanFang Database,VIP Database,China Biology Medicine disc(CBMdisc),PubMed,EMbase,Web of Science,and the Cochrane Library,from the establishment of databases until June 13,2022.After quality evaluation and data extraction,a Meta-analysis was performed using RevMan5.3 and Stata12.0 software.Results:A total of 14 pieces of literature were included,involving a total of 1381 patients.The main risk factors for pruritus in T2DM patients were:age[OR=1.28,95%CI(1.19,1.38),P<0.001],duration of disease[OR=1.13,95%CI(1.06,1.21),P=0.003],glycosylated hemoglobin[OR=1.71,95%CI(1.33,2.19),P<0.001],fasting blood glucose[OR=1.55,95%CI(1.21,1.99),P<0.001],dyslipidemia[OR=2.56,95%CI(1.99,3.29),P<0.001],diabetic retinopathy[OR=1.14,95%CI(1.07,1.21),P<0.001],and diabetic peripheral neuropathy[OR=1.37,95%CI(1.03,1.81),P=0.030].Conclusion:Advanced age,long disease duration,high glycosylated hemoglobin value,high fasting blood glucose value,abnormal blood lipid,diabetic retinopathy,and diabetic peripheral neuropathy were the main risk factors for pruritus in T2DM patients.The pathogenesis of diabetic nephropathy and smoking on pruritus was uncertain.
