基于数据挖掘分析中药内服治疗糖尿病足的用药规律

Analysis on the Rules of Medication for Diabetic Foot Treated by Oral Administration of Traditional Chinese Medicine Based on Data

目的基于数据挖掘分析中药内服治疗糖尿病足(DF)的用药规律。方法检索中国知网、万方和重庆维普三大中文数据库建库以来的相关文献,外文数据库Web of science、SpringerLink和Pubmed文献为补充;人工检索符合纳入标准的文献,并提取处方;统计分析药物的性味、归经、功效和系统可视化分析用药频次,筛选高频药物,并做关联规则分析和聚类分析。结果检索到4013篇文献,纳入279篇文献,319首处方,239味中药;高频药物依次是当归、黄芪、赤芍、川芎、丹参、甘草、红花等32味中药,多为补虚、活血化瘀、清热解毒、祛湿通络药;药物性味中温、平、寒、苦、甘、辛最多,多归为肝、脾、肺经;高频药物二项和三项关联规则中,支持度和置信度较高的是当归—红花、当归—川芎、当归—甘草、当归—赤芍和当归—川芎—黄芪;高频药物聚类分析提炼出5首新处方,由补阳还五汤、糖脉通、丹血蛭甲逍遥汤、当归四逆汤等经典处方加减而来。结论中药内服治疗DF整体处方以活血化瘀、益阳生阴为主,辅以清热解毒、祛湿生津和通络止痛,高频药物聚类分析提炼出5首新处方,为DF的治疗提供更多的可能性,亦起到基础研究和临床研究的桥梁作用。

Objective To analyze the medication rules of oral administration of traditional Chinese medicine for the treatment of diabetic foot(DF)based on data mining.Methods All literatures were retrieved by two researchers at the same time.The three major Chinese databases of CNKI,Wanfang and Chongqing VIP were the main ones,supplemented by the foreign language databases Web of Science,SpringerLink and Pubmed.The retrieval scope was since the establishment of the database.Manually searched literatures that meet the inclusion criteria,and extracted prescriptions.Statistically analyzed the aspects of the drug's nature,taste,meridian return,and efficacy,systematically and visually analyzed the frequency of medication,screened high-frequency drugs,and performed association rule analysis and cluster analysis.Results A total of 4013 literatures were retrieved,of which 279 literatures,319 prescriptions,and 239 traditional Chinese medicines met the inclusion criteria.The high-frequency drugs are 32 traditional Chinese medicines such as Root of Chinese Angelica,Root of Membranous Milkvetch,Red Peony root,Chuanxiong rhizome,root of Ligulilobe sage,Root of Ural Licorice,Safflower and so on.Their effects are mostly for tonifying deficiency,promoting blood circulation and removing blood stasis,clearing heat and detoxifying,removing dampness and dredging collaterals;The most frequent types of medicinal properties are warm,flat,cold,bitter,sweet and pungent,which are mostly classified as liver,spleen and lung meridians.Among the two-term and three-term association rules of high-frequency drugs,the ones with higher support and confidence are Root of Chinese Angelica-Safflower,Root of Chinese Angelica-Chuanxiong rhizome,Root of Chinese Angelica-Root of Ural Licorice,Root of Chinese Angelica-Red Peony root and Root of Chinese Angelica-Chuanxiong rhizome-Root of Membranous Milkvetch.A total of 5 new prescriptions have been extracted,through high-frequency drug cluster analysis,they are derived from the addition and subtraction of classic prescriptions such as Buyang Huanwu Decoction,Tangmaitong,Danxue Zhijia Xiaoyao Decoction,and Angelica Sini Decoction.Conclusion The overall prescription compatibility of traditional Chinese medicine for DF treatment by oral administration is mainly based on promoting blood circulation and removing blood stasis,invigorating yang and generating yin,supplemented by clearing heat and detoxifying,removing dampness and promoting body fluid,and dredging collaterals and relieving pain.5 new prescriptions for the treatment of DF have been refined from high-frequency drug cluster analysis,which will provide more possibilities for the treatment of DF.This research will serve as a bridge between basic research and clinical research.
