摘要
目的:初步探讨CT能谱成像单能量成像技术在肝门静脉CT血管成像扫描中的应用价值。方法:收集2021年1月~6月于上海市松江区中心医院行上腹部平扫及增强患者100例。将所有患者分为观察组与对照组两组,各50例,均行上腹部平扫及三期增强扫描。对照组患者行常规平扫及三期增强检查,观察组患者行CT能谱成像检查,门静脉期开启能谱模式。测量两组图像的门静脉的CT值、肝实质的CT值、门静脉噪声值标准差(SD)和皮下脂肪的SD值,计算计算门静脉的信噪比与门静脉与肝组织的对比噪声比(CNR)。记录两组患者的剂量长度乘积并计算其有效剂量。结果:门静脉与肝脏的最佳CNR能量约60keV。观察组重建图像在图像质量,门静脉CT值、肝实质CT值、门静脉的信噪比与门静脉与肝组织的CNR等方面均优于对照组(P<0.05)。对照组图像门静脉噪声低于观察组(P<0.05)。观察组受到的辐射剂量低于对照组(P<0.05)。结论:CT能谱成像单能量成像在肝门静脉CT血管成像检查中可以提高门静脉主干及分支的显影效果,使血管显示更清晰还能降低患者受到的辐射剂量。
Objective:To explore the application value of energy spectral CT single energy image(Mono)in CT portal imaging(CTPV)scanning.Methods:A total of 100 patients were collected in Shanghai from January to June 2021.All patients were divided into two groups:control group and observation group,and plain scan of the upper abdomen and three-stage enhanced scan were performed.Patients in control group underwent conventional plain scanning and third-stage enhancement examination,patients in observation group performed spectral CT examination,and the portal period.The CT value of the portal vein,the CT value of the liver parenchyma,the portal vein SD value and the SD value of the subcutaneous fat were measured,and the SNR of the portal vein and the CNR were calculated.DLP of patients in both groups were recorded and their ED calculated.Results:The optimal CNR energy between portal vein and liver was about 60 keV.The reconstructed images in observation group were better than control group in terms of image quality,portal vein CT value,liver parenchyma CT value,SNR of portal vein and CNR of portal vein and liver tissue(P<0.05).Image portal vein noise in control group was lower than that in observation group(P<0.05).The radiation dose received in observation group was lower than that in control group(P<0.05).Conclusion:The energy spectrum CT Mono can improve the development effect of the portal vein trunk and branches in CTPV examination,make the vascular display clearer and reduce the radiation dose of the patients.
