肝脓肿患者超声造影检查的临床特征及诊断价值

Clinical Characteristics and Diagnostic Value of Contrast-Enhanced Ultrasound in Patients with Hepatic Abscess

目的:探讨超声造影检查对肝脓肿患者的诊断价值及临床意义,以期为临床早期制定干预方案提供参考。方法:选取72例本院2019年8月~2021年8月就诊的疑似肝脓肿患者,入院后均行常规超声及超声造影检查,以手术或病理诊断结果为“金标准”,分析肝脓肿影像学表现,比较常规超声、超声造影检查诊断结果、诊断效能,并比较两种方案对不同时期肝脓肿的检出率。结果:经手术或病理诊断结果显示,72例疑似肝脓肿患者中共确诊54例,经常规超声检查结果显示共确诊43例,经超声造影检查结果显示共确诊52例;与常规超声比较,超声造影对于肝脓肿诊断灵敏度94.44%(51/54)、准确率94.44%(68/72)较高,漏诊率5.56%(3/54)较低(P<0.05);与常规超声比较,超声造影对于肝脓肿形成初期检出率较高(P<0.05)。结论:超声造影对于肝脓肿具有较高诊断价值,临床可通过超声造影进行早期筛查诊断,以降低漏诊、误诊现象发生,可尽早针对性制定治疗方案。

Objective:To investigate the diagnostic value and clinical significance of contrast-enhanced ultrasound in patients with liver abscess,so as to provide a reference for early clinical intervention.Methods:72 patients with suspected liver abscess in our hospital from August 2019 to August 2021 were selected.All patients underwent conventional ultrasound and contrast-enhanced ultrasound after admission.The results of surgical or pathological diagnosis were used as the"gold standard".The detection rates of liver abscess in different periods were compared between the two schemes.Results:According to surgical or pathological diagnosis,among 72 patients with suspected liver abscess,54 cases were diagnosed by operation or pathology,43 cases were diagnosed by conventional ultrasound,and 52 cases were diagnosed by contrast-enhanced ultrasound.Compared with conventional ultrasound,the sensitivity and accuracy of CEUS in the diagnosis of liver abscess were 94.44%(51/54)and 94.44%(68/72),respectively,and the missed diagnosis rate was 5.56%(3/54),which was lower(P<0.05).Compared with conventional ultrasound,contrast-enhanced ultrasound had a higher detection rate in the early stage of liver abscess formation(P<0.05).Conclusion:Contrast-enhanced ultrasound(CEUS)has a high diagnostic value for liver abscess.CEUS can be used for early screening and diagnosis in clinic,so as to reduce the occurrence of missed diagnosis and misdiagnosis,and make targeted treatment plan as soon as possible.
