超声检查联合血清Tg、TSH和CEA对PTC患者表达意义及诊断价值研究

Study on the Significance and Diagnostic Value of Ultrasound Combined with Serum Tg,TSH and CEA in Patients with PTC

目的:探究超声检查联合血清甲状腺球蛋白(Tg)、促甲状腺激素(TSH)和癌胚抗原(CEA)水平对甲状腺乳头状癌(PTC)患者表达意义及诊断价值。方法:回顾性纳入2017年3月~2022年3月本院收治的PTC患者30例作为研究对象,作为观察组,另外收集同期于本院就诊的健康人员30例作为对照组。采用GE二维超声模式对肿瘤情况进行测定并记录。采用化学发光免疫分析仪检测TSH、Tg和CEA水平。结果:观察组Tg、TSH和CEA表达均明显高于对照组,差异具有统计学意义(均P<0.05);对两组患者行超声检查,观察组检出符合率为83.33%,漏诊/误诊率为16.67%;对照组符合率为96.67%,漏诊/误诊率为3.33%,差异具有统计学意义(P<0.05);超声检查联合血清Tg、TSH和CEA的联合检测在PTC患者中诊断评估中的ROC曲线线下面积AUC为0.887,具有较高的特异性以及敏感度,且显著高于单独超声检查、血清Tg、TSH和CEA的ROC曲线线下面积,联合检测与单独检测方式分析预测模型有统计学差异(Z=2.533,P<0.05)。结论:对于PTC患者而言,对其应用超声联合Tg、TSH和CEA检查,可有效提升临床检出率,临床价值较高,可作为临床PTC重要检测方式。

Objective:To investigate the significance and diagnostic value of ultrasonography combined with serum thyroglobulin(Tg),thyroid stimulating hormone(TSH)and carcinoembryonic antigen(CEA)levels in the expression of patients with papillary thyroid cancer(PTC).Methods:30 patients with PTC from March 2017 to March 2022 in our hospital were retrospectively included as the study population and included in the study group,and an additional 30 healthy individuals who attended our hospital during the same period were collected as the control group.The tumor condition was measured and recorded using GE two-dimensional ultrasound mode.TSH,Tg and CEA levels were measured by chemiluminescent immunoassay analyzer.Results:The Tg,TSH and CEA expressions in the study group were significantly higher than those in the control group,with statistically significant differences(all P<0.05);Ultrasonography was performed in both groups,and the detection compliance rate in the study group was 83.33%,with a miss/misdiagnosis rate of 16.67%;The compliance rate in the control group was 96.67%,with a miss/misdiagnosis rate of 3.33%,with statistically significant differences(P<0.05).The AUC of the area under the ROC curve of the combined ultrasonography with serum Tg,TSH and CEA in the diagnostic evaluation of PTC patients was 0.887,which had higher specificity as well as sensitivity and was significantly higher than the area under the ROC curve of ultrasonography,serum Tg,TSH and CEA alone,and there was a statistically significant difference in the prediction model between the combined test and the analysis of the separate test modalities(Z=2.533,P<0.05).Conclusion:For patients with PTC,the application of ultrasound combined with Tg,TSH and CEA for them can effectively improve the clinical detection rate,which is of high clinical value and can be used as an important clinical PTC detection modality.
