经鼻无创高频振荡通气和经鼻间歇正压通气在早产儿撤机后的应用比较

摘要 目的:对经鼻无创高频振荡通气(NHFOV)与经鼻间歇正压通气(NIPPV)在早产儿撤机后的应用效果进行观察与比较。方法:选择2020年1月~2022年10月新乡市中心医院新生儿重症监护室收治的60例早产儿作为此次研究对象,60例早产儿中有30例属于NHFOV组,剩余30例属于NIPPV组;观察并记录两组研究对象无创通气过程中的血气指标;记录并分析两组研究对象撤机的成功率及其并发症的情况。结果:NHFOV组在24h、48h时pH值高于NIPPV组,两组数据具有统计学差异(P<0.05);NHFOV组在24h、48h时PaCO_(2)水平低于NIPPV组,两组数据具有统计学差异(P<0.05);在支气管肺发育不良发生率上,NHFOV组低于NIPPV组并且数据具有统计学差异(P<0.05)。结论:NHFOV应用在早产儿撤机后的无创呼吸支持治疗效果较好,撤机的成功比率较高,可降低支气管肺发育不良的发生率,是一种有效的无创辅助通气治疗方式。 Objective:To observe the effect of nasal non-invasive high frequency oscillatory ventilation(NHFOV)and nasal intermittent positive pressure ventilation(NIPPV)in premature infants after weaning from mechanical ventilation.Methods:60 premature infants admitted to the neonatal intensive care unit of Xinxiang Central Hospital from January 2020 to October 2022 were selected as the study subjects.Among the 60 premature infants,30 belonged to the NHFOV group,and the remaining 30 belonged to the NIPPV group;Observe and record the blood gas indicators of the two groups of study subjects during non-invasive ventilation;Record and analyze the success rate and complications of weaning in two groups of study subjects.Results:The pH value of the NHFOV group was higher than that of the NIPPV group at 24 and 48 hours,with statistical differences between the two groups(P<0.05);The PaCO_(2) levels in the NHFOV group were lower than those in the NIPPV group at 24 and 48 hours,with statistical differences between the two groups(P<0.05);In terms of the incidence of bronchopulmonary dysplasia,the NHFOV group was lower than the NIPPV group,and the data showed statistical differences(P<0.05).Conclusion:Compared with NIPPV,NHFOV as non-invasive respiratory support after weaning can improve the success rate of weaning,and reduce the incidence of BPD;It is an effective non-invasive assistant ventilation method.
