信息化血糖监测系统在医院多院区间血糖管理中的价值研究

Research on the value of informatized blood glucose management system in multi-campus of hospital blood glucose management

目的:探讨信息化血糖监测系统(IGMS)对医院不同院区间进行血糖管理的应用价值及其准确性与方便性。方法:在医院总部院区建立IGMS,通过局域网连接医院电子病历系统,对各科室的患者血糖进行信息化管理;以全自动生化分析仪和IGMS的智能血糖仪对抽取的5名志愿者5份静脉血样本进行检测对比,检测IGMS智能血糖仪的准确性;采用自制调研问卷对护士进行IGMS使用满意度调查,比较应用IGMS的智能血糖仪与非信息化便携式血糖仪的方便性。结果:医院总部院区56个科室全部完成IGMS安装,共监测11 656例患者血糖,检测血糖171 477次,其中低血糖(血糖<3.9 mmol/L)6 712次,占3.91%;高血糖(血糖>11.1mmo l/L)74 861次,占43.66%。在血糖为3.16 mmol/L时,IGMS的智能血糖仪血糖检测值100%在全自动生化分析仪检测值±0.83 mmol/L范围内;当血糖浓度≥4.2 mmol/L时,100%与全自动生化分析仪检测值偏差在±20%范围内。护士对IGMS的智能血糖仪的通知医生快速、记入病历系统方便、获得结果快和修正录入记录方便满意度分别为82.7%、86.5%、61.5%和82.7%,均高于非信息化便携式血糖仪,差异有统计学意义(χ^(2)=26.630,χ^(2)=42.275,χ^(2)=5.538,χ^(2)=12.446;P<0.05)。结论:IGMS可解决不同院区间无内分泌科室的院区血糖监测管理困难,且检测数据准确性好,护士应用体验满意度高,极为适合有多个分院区的医院使用。

Objective:To explore the application value,accuracy and convenience of the Informatized glucose management system(IGMS)in blood glucose management in inter-campus of the hospital.Methods:The IGMS was set up in the headquarter of the hospital and connecting the electronic medical record system through the local area network to carry out information management of blood glucose of patients in each department.The automatic biochemical analyzer and IGMS smart blood glucose meter were used to test and compare 5 samples to test the accuracy of the IGMS smart blood glucose meter.A self-made survey questionnaire was used to conduct a survey of nurses'satisfaction with using IGMS.The convenience of smart blood glucose meters using IGMS and non-information portable blood glucose meters was compared.Results:All 56 departments of the headquarter of the hospital completed the installation of IGMS.A total of 11,656 blood glucose patients were monitored,and blood glucose was tested 171,477 times,including 6,712 times of hypoglycemia(blood glucose<3.9 mmol/L),accounting for 3.91%;and 74,861 times of hyperglycemia(blood glucose>11.1mmo l/L),accounting for 43.66%.When the blood glucose was 3.16 mmol/L,the blood glucose detection value of the smart blood glucose meter of IGMS was 100%within the range of±0.83 mmol/L of the detection value of the automatic biochemical analyzer;when the blood glucose concentration was≥4.2 mmol/L,the deviation between 100%and the detection value of the automatic biochemical analyzer was within±20%.Nurses'satisfaction with IGMS smart blood glucose meter in terms of fast notification to the doctor,convenience in documentation system,quick results availability,and convenience in correcting records entry were 82.7%,86.5%,61.5%and 82.7%,respectively,which were higher than non-information portable blood glucose meters,and the difference was statistically significant(χ^(2)=26.630,χ^(2)=42.275,χ^(2)=5.538,χ^(2)=12.446;P<0.05).Conclusion:IGMS can solve the difficulty of inter-campus blood glucose monitoring and management without endocrinology departments.The accuracy of the test data is good,the satisfaction of the application of nurses is high,which is extremely suitable for a multi-campus hospital.
