人文关怀理念指导的家属参与零缺陷护理对老年骨折患者术后恢复的影响

Effect of family participatory zero-defect nursing guided by humanistic care theory on postoperative recovery of elderly fracture patients

目的探讨人文关怀理念指导的家属参与零缺陷护理对老年骨折患者术后恢复的影响。方法将2021年1~6月于江苏大学附属武进医院骨科就诊的120例老年骨折患者纳入研究,其中行常规护理的60例为对照组,行人文关怀理念指导的家属参与零缺陷护理60例为观察组。比较2组患者的恢复时间、Barthel指数、Harris髋关节评分、家庭关怀度指数问卷(APGAR)评分及并发症发生率。结果观察组的住院时间短于对照组,骨折愈合时间和完全承重时间均早于对照组(P均<0.05)。干预1个月和3个月后,2组的Barthel指数和Harris髋关节评分均比同组干预1周高(P均<0.05);干预3个月后,2组上述评分均比同组干预1个月高(P均<0.05)。观察组在干预1个月和3个月后的上述评分均较同期对照组更高(P均<0.05)。干预1周和3个月后,2组的APGAR评分均较入院时升高(P均<0.05);干预3个月后,2组的APGAR评分均比干预1周高(P均<0.05);观察组在干预1周和3个月后的APGAR评分均高于同期对照组(P均<0.05)。观察组的并发症总发生率比对照组低(P<0.05)。结论人文关怀理念指导的家属参与零缺陷护理模式可有效提高老年骨折患者的手术效果,改善日常活动度及髋关节功能,有效提升家庭关怀度,减少并发症发生。

Objective To explore the effect of family participatory zero-defect nursing guided by the humanistic care theory on the postoperative recovery of elderly fracture patients.Methods A total of 120 elderly fracture patients in the Department of Orthopedics at Wujin Hospital Affiliated to Jiangsu University from January to June 2021 were included in the study.The 60 patients who received routine nursing were set as the control group,and the 60 patients who received family participatory zero-defect nursing guided by the humanistic care theory were set as the observation group.The recovery time,Barthel Index,Harris Hip Score,Family APGAR(adaptability,partnership,growth,affection,and resolve)score,and complication rate of the two groups were compared.Results The hospitalization time,time to fracture healing,and time to complete weight bearing of the observation group were shorter than those of the control group(all P<0.05).After 1 and 3 months of intervention,the Barthel Index and Harris Hip Score of both groups were higher than those after 1 week of intervention(all P<0.05).After 3 months of intervention,the above scores of both groups were higher than those after 1 month of intervention(all P<0.05).The above scores in the observation group were higher than those in the control group after 1 and 3 months of intervention(all P<0.05).After 1 week and 3 months of intervention,the Family APGAR scores of both groups were higher than those at admission(all P<0.05).After 3 months of intervention,the Family APGAR scores of both groups were higher than those after 1 week of intervention(both P<0.05).The Family APGAR scores of the observation group were higher than those of the control group after 1 week and 3 months of intervention(both P<0.05).The total complication rate in the observation group was lower than that in the control group(P<0.05).Conclusion Family participatory zero-defect nursing model guided by the humanistic care theory can effectively improve the surgical outcomes,daily activities,and hip function of elderly fracture patients,effectively enhance the family care,and reduce the complications.
