某公司食堂一起甲胺磷中毒事件流行病学调查分析

Epidemiological investigation and analysis of a methamidophos poisoning incident in a company canteen

摘要 目的通过调查分析某公司食堂供应的午餐导致甲胺磷中毒事件,为防止此类事件再次发生提供参考。方法通过流行病学调查、现场卫生学调查和实验室检测收集资料,采用描述性流行病学方法进行分析。结果共25例发病,罹患率为9.26%(25/270),可能病例23例,确诊病例2例。潜伏期1 h~4.5 h,临床表现包括呕吐、多汗、头晕、腹痛、腹泻等,血液胆碱酯酶活性较参考下限值均下降30%以上,全部收治入院,平均住院时间为(5.12±2.15)d。2份病例呕吐物、1份食堂留样食品(咸菜毛豆肉片)、1份食堂食品原材料(咸菜存储桶中的剩余咸菜汁)检出甲胺磷,含量分别为10.9 mg/kg、14.6 mg/kg、2038 mg/kg、4272 mg/kg。结论这是一起由食用甲胺磷污染的咸菜毛豆肉片引起的食源性疾病暴发事件,应加大集体食堂的食品安全监督管理,加强农药使用管理,预防食源性农药中毒事件发生。 Objective This paper aims to investigate and analyze a methamidophos poisoning incident caused by lunch served in a company's canteen,so as to provide reference for preventing this kind of incidents.Methods The data was collected through epidemiological investigation,field hygienic investigation and laboratory test,and descriptive epidemiological methods were used for analysis.Results A total of 25 cases were found with an attack rate of 9.26%(25/270).There were 23 possible cases and 2 confirmed cases.The incubation period ranged from 1 h to 4.5 h.The clinical manifestations included vomiting,hyperhidrosis,dizziness,abdominal pain,diarrhea,etc.The activity of blood cholinesterase decreased by more than 30%compared with the reference minimum limit.All patients were admitted to hospital,and the average hospitalization time was(5.12±2.15)d.Methamidophos was detected in vomitus of two cases,one sample of food in canteen(sliced pork with pickles and soybeans)and one raw material of food in canteen(the remaining pickle juice in the storage bucket),with the contents of 10.9 mg/kg,14.6 mg/kg,2038 mg/kg and 4272 mg/kg,respectively.Conclusion This is an incident of foodborne diseases caused by the consumption of methamidophos contaminated sliced pork with pickles and soybeans.Food safety supervision and management in collective canteens should be strengthened,and pesticide management should be strengthened to prevent foodborne pesticide poisoning.
