Effect of Invisible Correction without Bracket on Periodontitis with Malocclusion

目的:探讨无托槽隐形矫治治疗牙周炎伴错畸形的效果。方法:选择2020年1月—2021年12月福建医科大学附属厦门弘爱医院收治的105例牙周炎伴错畸形患者,采用随机数表法分为试验组(n=53)和对照组(n=52)。对照组给予直丝弓矫治器治疗,试验组给予无托槽隐形矫治治疗。比较两组的疗效、牙龈出血指数、牙周袋探诊深度、牙周炎症情况及并发症发生情况。结果:治疗后,试验组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,两组牙龈出血指数、牙周袋探诊深度均较治疗前升高,但试验组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组牙周炎症情况比较,差异无统计学意义(P>0.05);治疗后,两组的肿瘤坏死因子-α(TNF-α)、白细胞介素-5(IL-5)、白细胞介素-8(IL-8)及白细胞介素-1β(IL-1β)水平均较治疗前升高,但试验组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组咀嚼、语言功能评分均较治疗前升高,且试验组高于对照组,差异有统计学意义(P<0.05)。治疗后,试验组发音功能、舒适度及整齐美观感满意度高于对照组,固位能力低于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:在牙周炎伴错畸形治疗期间采取无托槽隐形矫治效果显著,可有效改善患者出血指数,牙周袋探诊深度水平。

Objective:To explore the effect of periodontitis with malocclusion by the invisible correction without bracket.Method:A total of 105 periodontitis patients with malocclusion treated in Xiamen Hong’ai Hospital Affiliated to Fujian Medical University from January 2020 to December 2021 were selected,and they were divided into experimental group(n=53)and control group(n=52)by random number table method.The control group was treated with straight wire appliance,and the experimental group was treated with invisible correction without bracket.The curative effect,gingival bleeding index,probing depth of periodontal pocket,periodontitis and complications were compared between the two groups.Result:After treatment,the total effective rate of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the gingival bleeding index and probing depth of periodontal pocket in the two groups were higher than those before treatment,but the experimental group were lower than those in the control group,the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in the periodontal inflammation between the two groups(P>0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-5(IL-5),interleukin-8(IL-8)and interleukin-1β(IL-1β)in both groups were higher than those before treatment,but the levels in the experimental group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the scores of chewing and language function in the two groups were higher than those before treatment,and the scores in the experimental group were higher than those in the control group,the differences were statistically significant(P<0.05).After treatment,the degree of satisfaction with pronunciation function,comfort and tidiness in the experimental group were higher than those in the control group,and the retention ability was lower than that in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:During the treatment of periodontitis with malocclusion,the effect of invisible correction without bracket is significant,which can effectively improve the bleeding index of patients and the level of probing depth of periodontal pocket.
