多发性骨髓瘤患者血清中miR-223-3p和miR-140-5p表达水平与肾功能损害的相关性分析

摘要 目的探讨血清miR-223-3p和miR-140-5p表达水平与多发性骨髓瘤患者肾功能损害的相关性。方法选取本院于2019年7月—2022年6月收治的165例多发性骨髓瘤患者,按照入院时诊断结果中存在肾损伤与未见肾损伤分为观察组与对照组。比较2组血清中血肌酐(Scr)、血尿素氮(BUN)、24 h尿蛋白水平以及胱抑素C(Cys-C),检测血清miR-223-3p和miR-140-5p表达水平,分析血清miR-223-3p和miR-140-5p表达水平与多发性骨髓瘤患者肾功能损害的相关性。结果与对照组比较,观察组患者血清miR-223-3p和miR-140-5p表达水平均降低(P<0.05),血清中Scr、BUN、24 h尿蛋白以及Cys-C水平升高(P<0.05);不同性别、年龄、肿瘤细胞形态、患者血清miR-223-3p、miR-140-5p表达水平差异无统计学意义(P>0.05),低肿瘤分化及与Ⅲ期患者血清miR-223-3p、miR-140-5p表达水平低于高、中分化及Ⅰ期、Ⅱ期患者(P<0.05);Pearson相关性分析结果显示,血清miR-223-3p、miR-140-5p水平与Scr、BUN、24 h尿蛋白水平均成负相关(P<0.05)。结论多发性骨髓瘤患者血清miR-223-3p、miR-140-5p表达与肿瘤分期和肿瘤分化进程密切相关,并可反映患者肾功能损伤程度。 Objective This paper aims to investigate the correlation between the levels of serum miR-223-3p and miR-140-5p and renal function impairment in patients with multiple myeloma.Methods A total of 165 patients with multiple myeloma who were admitted to our hospital from July 2019 to June 2022 were selected and divided into the observation group and the control group according to the presence or absence of renal injury in the diagnosis results at admission.The serum levels of serum creatinine(Scr),blood urea nitrogen(BUN),24-hour urine protein and cystatin C(Cys-C)were compared among the three groups,and the levels of serum miR-223-3p and miR-140-5p were detected.The correlation of miR-223-3p and miR-140-5p levels with renal impairment in patients with multiple myeloma was analyzed.Results Compared with the control group,the levels of serum miR-223-3p and miR-140-5p in the observation group were decreased(P<0.05),and the levels of Scr,BUN,24 h urine protein and Cys-C in serum were increased(P<0.05);there was no statistical significance on the difference in the levels of serum miR-223-3p and miR-140-5p in patients with different genders,ages and tumor cell morphologies(P>0.05).The levels of miR-223-3p and miR-140-5p in patients with low tumor differentiation and stageⅢwere lower than those in patients with high tumor differentiation and stageⅠand stageⅡ(P<0.05);Pearson correlation analysis showed that serum miR-223-3p and miR-140-5p levels were negatively correlated with Scr,BUN,and 24 h urine protein levels(P<0.05).Conclusion The levels of serum miR-223-3p and miR-140-5p in patients with multiple myeloma are closely related to tumor stage and tumor differentiation process,and can reflect the degree of renal function damage in patients.
