摘要
目的探讨慢性阻塞性肺疾病(COPD)肺部感染易感性与白细胞介素-17(IL-17)基因多态性的关系。方法选取2020年6月-2021年6月医院诊治的79例COPD肺部感染患者(COPD肺部感染组)及45例无肺部感染COPD患者(COPD组)为研究对象,检测两组IL-17基因多态性、外周血IL-17水平和肺功能指标变化,分析IL-17基因多态性与COPD肺部感染的关系。结果IL-17基因rs12203582、rs1266828、rs7771466、rs2275913、rs3819024、rs4711998位点分布在两组中均符合Hardy-Weinberg遗传平衡定律(P>0.05)。IL-17基因位点rs2275913的AA、GG型、rs3819024的AA、GG型与COPD肺部感染相关,COPD肺部感染组患者rs2275913、rs3819024位点在显性模型、加性模型下与肺部感染相关(P<0.05)。COPD肺部感染组外周血IL-17水平较COPD组高,第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、第1秒用力呼气容积占预计值百分比(FEV1%)较COPD组低(P<0.05),COPD肺部感染患者外周血IL-17水平与肺功能指标呈相关性(P<0.05)。结论IL-17基因多态性与COPD肺部感染易感性相关,临床可通过检验患者IL-17基因多态性及外周血IL-17水平评估COPD肺部感染发生风险。
OBJECTIVE To investigate the relationship between the susceptibility to pulmonary infection in chronic obstructive pulmonary disease(COPD)and peripheral blood interleukin-17(IL-17)gene polymorphism.METHODS Total of 79 patients with COPD and pulmonary infection,and 45 patients with COPD but without pulmonary infection who were admitted to the hospital from Jun 2020 to Jun 2021 were enrolled as the subjects.IL-17 gene polymorphism,variation levels of IL-17 in peripheral blood and lung function indicators between the two groups were detected.The relationship between pulmonary infection in COPD and IL-17 gene polymorphism was analyzed.RESULTS The distribution of rs12203582,rs1266828,rs7771466,rs2275913,rs3819024 and rs4711998 of IL-17 gene between the two groups was consistent with hardy-weinberg equilibrium law(P<0.05).AA and GG genotypes of IL-17 gene at rs2275913 and rs3819024 sites were associated with pulmonary infection in COPD.Rs2275913 and rs3819024 sites in the COPD with pulmonary infection group were associated with pulmonary infection under dominant and additive models(P<0.05).Peripheral blood IL-17 level in the COPD with pulmonary infection group was higher than that in the COPD group,while forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC)and the percentage of FEV1 in the predicted value(FEV1%)were lower than those in the COPD group(P>0.05).There was a significant correlation between peripheral blood IL-17 level and lung function indicators in patients with COPD and pulmonary infection(P<0.05).CONCLUSION IL-17 gene polymorphism is related to susceptibility to pulmonary infection in COPD.Clinically,the risk for pulmonary infection can be assessed by the detection of peripheral blood IL-17 gene polymorphism and peripheral blood IL-17 level.
