胰岛素、低分子肝素、非诺贝特联合治疗与序贯治疗高脂血症性胰腺炎的临床效果比较

Comparison of Clinical Effect of Insulin,Low Molecular Weight Heparin and Fenofibrate Combined Therapy and Sequential Therapy for Hyperlipidemic Pancreatitis

目的:比较胰岛素、低分子肝素、非诺贝特联合治疗与序贯治疗高脂血症性胰腺炎(HLP)的临床疗效。方法:选取2018年1月2020年12月深圳市宝安区人民医院收治的HLP患者56例作为研究对象,根据随机数字表法分为对照组与观察组,各28例。观察组给予胰岛素、低分子肝素、非诺贝特序贯治疗,对照组给予胰岛素、低分子肝素、非诺贝特联合治疗。比较两组患者血清甘油三酯(TG)、淀粉酶、急性生理与慢性健康评分、胰腺炎严重程度分级、预后。结果:治疗第7天,观察组血清TG水平高于对照组,差异有统计学意义(P=0.010)。结论:尽早使用口服降脂药物,可以稳定控制TG。

Objective:To compare the clinical efficacy of insulin,low molecular weight heparin and fenofibrate combined therapy and sequential therapy for hyperlipidemic pancreatitis(HLP).Methods:Fifty-six patients with HLP admitted to Shenzhen Bao'an District People's Hospital from January 2018 to December 2020 were selected as the study subjects.According to the random number table method,they were divided into control group and observation group,with 28 cases in each groups.The observation group was given sequential therapy of insulin,low molecular weight heparin and fenofibrate,and the control group was given combined therapy of insulin,low molecular weight heparin and fenofibrate.Serum triglycerides(TG),amylase,acute physiological and chronic health evaluation scores,grade of pancreatitis severity,and prognosis were compared between the two groups.Results:On Day 7 of treatment,the serum TG level in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.010).Conclusion:Early use of oral lipid-lowering drugs can stabilize and control TG.
