口服荧光素共焦激光眼底血管造影的临床应用

Clinical application of oral fluorescein angiography with the scanning laser ophthalmoscope in patients with a history of drug allergy

目的观察口服荧光素钠共焦激光眼底血管造影在有药物过敏史患者中的安全性和有效性。方法回顾性病例研究。分析2018年1月至2019年12月行口服荧光素钠共焦激光眼底血管造影检查的有药物过敏史的患者42例。结果口服荧光素钠眼底显影时间平均(21.56±6.37)min,与静脉注射荧光素钠组相比,差异有统计学意义(P=0.004);口服荧光素钠1.0 g后未出现不良反应,与静脉注射荧光素组相比,两组不良反应的差异有统计学意义(P=0.006);两组视网膜血管识别分级、图像质量分级差异无统计学意义(P=0.123、P=0.248)。口服荧光素眼底显影时间与体重相关(r=0.707,P<0.001)。结论对于静脉注射有禁忌或者有顾虑的有药物过敏史的患者,口服荧光素钠共焦激光眼底血管造影可为一种有效安全的替代。

Objective To observe the safety and effectiveness of oral fluorescein sodium confocal laser fundus angiography in patients with a history of drug allergy.Methods This retrospective analysis enrolled 42 cases with a history of drug allergy who had oral fluorescein sodium confocal laser fundus angiography from January 2018 to December 2019.Results The average time for dye entry into the retinal circulation after ingestion in Oral-FFA group was 21.56±6.37 minutes.No adverse reactions occurred after ingestion 1.0 g fluorescein sodium.Compared with the intravenous fluorescein group,the difference in adverse reactions between the two groups was statistically significant(P<0.05).Compared with the intravenous fluorescein group,the retinal blood vessel recognition classification and image quality classification were not statistically different(P>0.05).The time for dye entry into the retinal circulation after ingestion in Oral-FFA group was correlated with body weight(r=0.707,P<0.001).Conclusions For patients with a history of drug allergy who was contraindicated or concerned about intravenous injection,oral fluorescein sodium confocal laser fundus angiography can be an effective and safe alternative.
