肝癌手术患者创伤后成长水平调查及其与社会支持的关系分析

Investigation of posttraumatic growth level of patients with liver cancer surgery and analysis of its relationship with social support

目的了解肝癌手术患者创伤后成长(posttraumatic growth,PTG)、心理弹性水平、社会支持度,分析PTG与社会支持的相关性,为改善肝癌手术患者的情绪提供参考。方法采用随机抽样法,运用创伤后成长量表(posttraumatic growth inventory,PTGI)、社会支持评定量表(social support rating scale,SSRS)和心理弹性量表(connor davidson-resilience scale,CD-RISC)对225例肝癌手术患者的PTG水平、社会支持度及心理弹性进行调查,采用Pearson相关性分析患者PTG水平和社会支持之间的关系。结果肝癌手术患者PTGI总得分29~85分,平均(58.84±8.80)分,PTG的阳性检出率为55.11%(124/225);SSRS总得分为19~46分,平均(35.16±3.28)分。PTG阳性组与阴性组患者在年龄、病程、家庭人均月收入、文化程度、居住地、疾病分期上,差异均有统计学意义(P<0.05);阳性组PTGI总分及各因子得分、SSRS总分及各维度得分均显著高于阴性组,差异均有统计学意义(P<0.05)。随着社会支持度的升高,患者的CD-RISC总分及各维度得分也随之升高(P<0.05);肝癌手术患者的SSRS总分与PTGI总分呈正相关(P<0.05)。结论肝癌手术患者PTG水平大多处于中等偏下,PTG水平较低的患者心理弹性和社会支持度均较低,临床可进行针对干预,以改善肝癌手术患者的情绪状态。

Objective To understand the posttraumatic growth(PTG),psychological resilience,and social support of patients with liver cancer surgery and analyze the correlation between PTG and social support so as to provide reference for improving the mood of patients with liver cancer surgery.Methods A random sampling method was used to investigate the PTG level,social support,and psychological resilience of 225 patients with liver cancer surgery using Posttraumatic growth inventory(PTGI),social support rating scale(SSRS),and Connor davidson-resistance scale(CD-RISC).Pearson correlation was used to analyze the relationship between the PTG level and social support.Results The total score of PTGI in patients with liver cancer surgery was 29-85 points,with an average of(58.84±8.80)points.The positive detection rate of PTG was 55.11%(124/225).The total score of social support was 19-46 points,with an average of(35.16±3.28)points.There were statistically significant differences between the PTG positive group and the negative group in age,course of disease,per capita monthly income of households,education level,residence,and disease stage(P<0.05).The total score of PTGI and scores of all factors,the total score of social support and scores of all dimensions in the positive group were significantly higher than those in the negative group.The differences were statistically significant(P<0.05).With the increase of social support,the total score of CD-RISC scale and the scores of all dimensions of patients also increased(P<0.05).The total score of social support in patients with liver cancer surgery was positively correlated with the total score of PTG(P<0.05).Conclusions The PTG level of patients with liver cancer surgery is mostly lower than the middle level,and the patients with lower PTG level have lower psychological resilience and social support.Clinical intervention can be targeted to improve the emotional state of patients with liver cancer surgery.
