腹部肌力动作用于产后腹直肌分离超声诊断可行性研究

The feasibility of performing abdominal crunch according to manual muscle testing in diagnosis of diastasis recti abdominis postpartum by ultrasound

目的:探讨将腹部肌肉徒手肌力分级(Manual muscle testing,MMT)卷腹动作应用于超声测量腹直肌间距(Inter recti distance,IRD)诊断腹直肌分离的可行性。方法:前瞻性、多中心收集产后42~63天于东南大学附属中大医院及无锡市锡山人民医院超声科行产后腹直肌分离超声检查的初产妇,比较同一研究中心不同肌力产妇及不同研究中心相同肌力产妇行卷腹动作时IRD变化量,初步判断腹部肌肉MMT卷腹动作应用于超声测量IRD是否具有可行性。结果:不同肌力产妇使用相应MMT卷腹动作时的IRD变化量无显著差异(P=0.525),各研究中心操作者间对相同肌力的2~4级肌力产妇使用相应MMT卷腹动作时IRD变化量无显著差异(P>0.05)。结论:腹部肌肉MMT卷腹动作应用于超声测量IRD在不同肌力产妇间及不同检查者间具有较好的一致性,因此具有较高的可行性及临床应用价值。

Objective:To investigate the feasibility of performing abdominal crunch according to manual muscle testing(MMT)in measuring inter recti distance(IRD)and diagnosing diastasis recti abdominis.Methods:This was a prospective multi-center study of women who underwent ultrasound examination of diastasis recti abdominis 42~63 days after delivery at Zhong-da Hospital Southeast University and Xishan People’s Hospital of Wuxi.The change of IRD during abdominal crunch in women of different abdominal muscle strength or different hospital were compared to confirm the feasibility of performing ab-dominal crunch according to MMT in measuring IRD by ultrasound.Results:The change of IRD in women with different ab-dominal muscle strength during abdominal crunch has no statistical significant difference(P=0.525).The change of IRD in women with the same abdominal muscle strength of level 2 to 4 in different hospital has no statistical significant difference(P>0.05).Conclusion:Performing abdominal crunch according to MMT of abdominal muscles is feasible and of high clinical val-ue in women of different abdominal muscle strength and different examiners.
