钱秋海教授辨证治疗痛风经验总结

Professor QIAN Qiuhai’s experience in treating gout based on syndrome differentiation

痛风是一种常见的慢性病,主要原因是单钠尿酸盐在关节及组织间沉积形成的急慢性炎症。其不仅与年龄、性别、饮食、生活方式有关,遗传因素也起着重要作用。随着现代生活的进步,痛风的发病率越来越高,发病年龄逐渐低龄化。钱秋海教授从事中医临床工作多年,在高尿酸血症、痛风的诊治方面有独特的学术观点,并取得了显著的疗效。钱秋海教授认为,痛风发病病因为外感和内伤,治疗时不仅要注重外因,更要重视祛内邪,其痰、火、瘀等病理产物是引起痛风反复发作的根本,应根据患者实际情况进行加减用药。治疗中应抓主要症状,准确辨证,方症对应,经方只要运用得当,在辨证准确的基础上方症对应,便能迅速见效。运用经方辨证论治,对于痛风的治疗效果显著,在促进尿酸排泄、缓解疼痛、预防疾病进展方面都有显著收益。文章从痛风的病因病机、分期、辨证论治、理法方药等多方面对钱秋海教授的治疗经验进行总结,以求为临床辨治痛风提供新的思路。

Gout is a common chronic disease,mainly caused by acute and chronic inflammation caused by the deposition of monosodium urate between joints and tissues.It is not only related to age,gender,diet,lifestyle,but also genetic factors play an important role.With the progress of modern life,the incidence of gout is getting higher and higher,and the age of onset is gradually younger.Professor QIAN Qiuhai has been engaged in clinical work of TCM for many years,has his own unique academic viewpoints in the diagnosis and treatment of hyperuricemia and gout,and has achieved remarkable curative effects.Professor QIAN Qiuhai believes that the onset of gout is due to external sensation and internal injury,and the treatment should not only pay attention to external causes,but also pay attention to dispelling internal evil,and its phlegm,fire,stasis and other pathological products are the root cause of repeated attacks of gout.And the decoction should be added or subtracted according to the actual situation of patients.In the treatment,we should grasp the main symptoms,accurately distinguish,and respond to the symptoms.As long as the prescription is applied properly,on the basis of accurate differentiation,the effect can be achieved quickly.Application of classical decoction by differentiation has a significant effect on the treatment of gout,and has significant benefits in promoting uric acid excretion,relieving pain,and preventing disease progression.In this paper,Professor QIAN Qiuhai’s clinical experience is summarized from the aspects of etiology and pathogenesis,disease stage,syndrome differentiation and treatment,and theory-method-prescription-medicine of gout,in order to provide new ideas for clinical diagnosis and treatment of gout.
