血清铁蛋白及红细胞参数在孕产妇缺铁性贫血中的诊断价值研究

Study on the diagnostic value of serum ferritin and red blood cell parameters in iron-deficiency anemia of pregnant and lying-in women

目的探讨血清铁蛋白及红细胞参数在孕产妇缺铁性贫血(IDA)中的诊断价值。方法选取2020年10月—2021年3月期间赣州市南康区第二人民医院收治的IDA孕产妇54例作为观察组,同期正常孕产妇54例作为对照组。对比2组血清铁蛋白(SF)、红细胞平均体积(MCV)、平均红细胞血红蛋白含量(MCH)、平均红细胞血红蛋白浓度(MCHC)以及红细胞体积分布宽度(RDW),分析SF与红细胞参数对孕产妇IDA的诊断效能。结果观察组的SF、MCV、MCH、MCHC水平均比对照组低,而RDW水平比对照组高,差异具有统计学意义(P<0.05)。单独检测孕产妇IDA时,SF的灵敏度和特异度明显高于各项红细胞参数,而RDW的灵敏度和特异度明显高于其他红细胞参数。将SF和RDW联合检测时,诊断IDA的灵敏度为90.22%,特异度为96.45%,明显比单独检测高。结论血清SF以及红细胞参数在诊断孕产妇IDA中具有一定的指导作用,其中血清SF联合红细胞参数RDW可以明显提高灵敏度和特异度,具有重要的临床诊断价值。

Objective To investigate the diagnostic value of serum ferritin and red blood cell parameters in iron-deficiency anemia(IDA)in pregnant women.Methods 54 IDA pregnant women who were admitted to our hospital from October 2020 to March 2021 were selected as the observation group,and 54 normal pregnant women during the same period were selected as the control group.The two groups of subjects were tested for serum ferritin and red blood cell parameters,compare and analyze the difference of test results.The red blood cell parameters included mean red blood cell volume(MCV),mean red blood cell hemoglobin content(MCH),mean red blood cell hemoglobin concentration(MCHC),and red blood cell volume distribution width(RDW).Results The SF,MCV,MCH,MCHC levels of the observation group were lower than those of the control group,while the RDW levels were higher than that of the control group,and the differences between the two groups were statistically significant(P<0.05).When the IDA of pregnant and lying-in women was detected alone,the sensitivity and specificity of SF were significantly higher than the red blood cell parameters,and the sensitivity and specificity of RDW in the red blood cell parameters were significantly higher than other red blood cell parameters.When SF and RDW are combined detection,the sensitivity of IDA detection is 90.22%,and the specificity is 96.45%,which is significantly higher than the detection alone.Conclusion Serum SF and red blood cell parameters have a certain guiding role in the diagnosis of maternal IDA.The combination of serum SF and red blood cell parameters RDW can significantly improve the sensitivity and specificity of diagnosis,and has important clinical diagnostic value.
