郑惠芳辨治子宫肌瘤经验

Experience of Zheng Huifang in the Treatment of Uterine Fibroids

郑惠芳教授结合数十载临证经验,认为子宫肌瘤发病与肝、脾、肾密切相关,病位为胞宫及胞络,病机为脏腑气阴两虚、胞络痰瘀痼结。提出治疗子宫肌瘤的“三并举”:扶正与祛邪并举、活血与止血并举、化瘀与消痰并举。治疗时提倡消补兼施,以益气养阴、化瘀消癥为主要治法;并创立分期治疗,平素以益气养阴、扶正补血为主以治崩,经期以化瘀消癥、软坚散结为主以治瘕。注重调和阴阳、疗养摄生,将郑氏妇科温补思想贯穿始终,自拟参芪龙牡汤,随证加减,获效甚捷。

Based on decades of clinical experience,Professor Zheng Huifang believes that the incidence of uterine fibroids is closely related to the liver,spleen and kidney,that the location of the disease is uterus and uterine collaterals,that the pathogenesis is deficiency of both qi and yin in zang-fu organs,as well as phlegm and blood stasis in uterine collaterals.She proposes the“three simultaneous therapies”for the treatment of uterine fibroids:simultaneous treatment of strengthening healthy qi and eliminating pathogenic factors,simultaneous treatment of promoting blood circulation and hemostasis,and simultaneous treatment of removing blood stasis and eliminating phlegm.During the treatment,the combination of eliminating and tonifying is advocated,with the main treatments of replenishing qi to nourish yin and resolving stasis to eliminate disease.She also establishes the staging therapy,which is usually based on tonifying qi to nourish yin,strengthening healthy qi to tonify blood to treat metrorrhagia;in the menstrual period,the main treatment is removing blood stasis to eliminate disease,softening and disintegrating mass.She also pays attention to harmonizing yin and yang,recuperating and taking care of health,putting Zheng’s gynecological warming and nourishing thoughts through the whole process.Besides,self-made Shenqi Longmu Decoction,which is modified according to the syndrome,has achieved confirmed efficacy.
