NBI电子喉镜及血清ANXA2、CCL2联合诊断早期喉癌的价值 被引量:2

Value of NBI electronic laryngoscope combined with serum ANXA2 and CCL2 in the diagnosis of early laryngeal cancer

摘要 目的探讨窄带成像技术(NBI)电子喉镜及血清膜联蛋白A2(ANXA2)CC型修饰趋化因子配体2(CCL2)联合诊断早期喉癌的价值。方法回顾性选取2020年7月至2022年7月青岛市城阳区人民医院收治的喉部病变患者78例作为研究对象。入院后均行NBI电子喉镜检查及病理检查,依据病理检查结果将78例患者分为恶性组(n=56)和良性组(n=22)。比较NBI电子喉镜检查结果和病理检查结果;通过一致性分析NBI电子喉镜检查诊断早期喉癌的价值;比较两组患者血清ANXA2、CCL2水平;通过受试者工作特征(ROC)分析血清ANXA2、CCL2诊断早期喉癌的价值;通过一致性分析NBI电子喉镜及血清ANXA2、CCL2联合诊断早期喉癌的价值。结果78例患者均为单发病灶,其中病理性质为阳性(恶性)的有56例,阴性(良性)的有22例,NBI电子喉镜检查为阳性的有51例,阴性的有27例。经一致性分析,78例早期喉癌中NBI电子喉镜诊断50例阳性,敏感度为89.29%,特异度为95.45%,准确率为91.03%,Kappa=0.793。恶性组患者血清ANXA2、CCL2水平为(45.13±8.76)、(103.44±22.57)pg/mL,明显高于良性组[(20.86±2.43)、(69.33±15.24)pg/mL],差异均有统计学意义(P<0.05)。经ROC分析血清ANXA2、CCL2检查诊断早期喉癌的曲线下面积分别为0.916、0.880,诊断价值较好(P<0.05)。经一致性分析,78例早期喉癌中联合诊断55例阳性,敏感度为98.21%,特异度为90.91%,准确率为96.15%,Kappa=0.904。结论早期喉癌患者通过NBI电子喉镜检查可获得较高的准确率,同时早期喉癌患者血清ANXA2、CCL2水平呈现出明显增高,另外经一致性分析证实NBI电子喉镜及血清ANXA2、CCL2联合诊断早期喉癌可获得较高的准确率。 Objective To investigate the value of narrowband imaging(NBI)electronic laryngoscope and serum annexin A2(ANXA2)CC type modified chemokine ligand 2(CCL2)in the diagnosis of early laryngeal cancer.Methods A tota of 78 patients with laryngeal diseases admitted to Chengyang District People's Hospital from July 2020 to July 2022 were retrospectively selected as the study subjects.After admission,all patients underwent NBI electronic laryngoscopy and pathological examination.According to the results of pathological examination,78 patients were divided into malignant group(n=56)and benign group(n=22).The results of NBI electronic laryngoscopy and pathology were compared;the value of NBI electronic laryngoscopy in the diagnosis of early laryngeal cancer was analyzed by consistency;the levels of serum ANXA2 and CCL2 were compared between the two groups;the diagnostic value of serum ANXA2 and CCL2 in early laryngeal cancer was analyzed by receiver operating characteristic(ROC);the value of NBI electronic laryngoscope combined with serum ANXA2 and CCL2 in the diagnosis of early laryngeal cancer was analyzed by consistency.Results 78 patients were all single lesions,of which 56 were positive(malignant),22 were negative(benign),51 were positive and 27 were negative by NBI electronic laryngoscopy.According to the consistency analysis,50 of 78 cases of early laryngeal cancer diagnosed by NBI electronic laryngoscope were positive,the sensitivity was 89.29%,the specificity was 95.45%,the accuracy was 91.03%,Kappa=0.793.The levels of serum ANXA2 and CCL2 in malignant group were(45.13±8.76),(103.44±22.57)pg/mL,which were significantly higher than those in benign group[(20.86±2.43),(69.33±15.24)pg/mL],the differences were statistically significant(P<0.05).The area under the curve of serum ANXA2 and CCL2 detected by ROC analysis for early laryngeal cancer was 0.916 and 0.880 respectively,with good diagnostic value(P<0.05).According to the consistency analysis,55 out of 78 cases of early laryngeal cancer were diagnosed as positive,the sensitivity was 98.21%,the specificity was 90.91%,the accuracy was 96.15%,and Kappa=0.904.Conclusion The patients with early laryngeal cancer can obtain a higher accuracy rate through NBI electronic laryngoscopy,and the levels of serum ANXA2 and CCL2 in patients with early laryngeal cancer show a significant increase.In addition,the consistency analysis confirmed that the combination of NBI electronic laryngoscopy and serum ANXA2 and CCL2 can obtain a higher accuracy rate in diagnosing early laryngeal cancer.
