关节腔药物注射疗法联合针刺对肩关节周围炎患者肩关节活动度的影响

The Influence of Intra-articular Drug Injection Therapy Combined with Acupuncture on Shoulder Joint Range of Motion in Patients with Shoulder Periarthritis

目的:观察关节腔药物注射疗法联合针刺对肩关节周围炎(简称肩周炎)患者肩关节活动度的影响。方法:选取2020年2月至2021年10月上海市交通大学医学院附属仁济医院宝山分院门诊收治的肩周炎患者120例,按照随机数字表法分为观察组和对照组,每组60例。对照组给予关节腔药物注射疗法治疗,观察组在对照组治疗的基础上给予针刺治疗。观察两组患者临床疗效及治疗前后肩关节活动度、疼痛程度、肌力及日常生活活动能力变化情况。结果:观察组有效率为96.7%,对照组有效率为81.7%,观察组有效率高于对照组,差异具有统计学意义(P<0.05);两组患者治疗后后伸、前屈、内收、外展、外旋等方位肩关节活动度均大于治疗前,且观察组治疗后肩关节各方位活动度高于对照组,差异具有统计学意义(P<0.05);观察组治疗后日常生活活动能力量表评分高于对照组,疼痛视觉模拟评分低于对照组,差异有统计学意义(P<0.05);观察组治疗后肩关节功能及肌力改善程度优于对照组,差异具有统计学意义(P<0.05)。结论:关节腔药物注射疗法联合针刺能缓解肩周炎患者临床症状,促进关节功能恢复,有效减轻疼痛程度,提高患者日常生活能力。

Objective:To observe the influence of intra-articular drug injection therapy combined with acupuncture on the range of motion of shoulder joint in patients with shoulder periarthritis.Methods:A total of 120 patients with shoulder periarthritis treated in Baoshan Branch of Renji Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from February 2020 to October 2021 were randomly divided into the observation group and the control group,with 60 cases in each group.The control group was treated with intra-articular drug injection,while the observation group was treated with acupuncture on the basis of the control group.The clinical efficacy and the changes of shoulder joint range of motion,pain degree,muscle strength and activities of daily living before and after treatment were observed.Results:The effective rate of the observation group was 96.7%,which was significantly higher than that of the control group:81.7%,and the difference was statistically significant(P0.05).The range of motion of the shoulder joint such as back extension,forward flexion,abduction,and external rotation,etc.in the two groups after treatment was greater than that before treatment,and the range of motion of the shoulder joint in the observation group after treatment was higher than that in the control group,and the difference was statistically significant(P0.05).After treatment,the scores of activities of daily living scale in the observation group were higher than those in the control group,and the pain visual analog scores were lower than those in the control group,and all the differences were statistically significant(P0.05).The improvement of shoulder joint function and muscle strength in the observation group was better than that in the control group after treatment,and the difference was statistically significant(P0.05).Conclusion:Intra-articular drug injection therapy combined with acupuncture can alleviate the clinical symptoms of patients with periarthritis of shoulder,promote the recovery of joint function,effectively reduce the degree of pain,and improve the ability of daily living of patients.
