计算机辅助训练对卒中后认知障碍患者认知功能康复效果的Meta分析

Meta analysis of the computer assisted cognitive training for post stroke cognitive impairment patients

摘要 目的评价计算机辅助认知训练对卒中后认知障碍(post-stroke cognitive impairment,PSCI)患者认知功能的影响。方法利用PubMed、Web of Science、CNKI、万方等数据库,检索有关计算机辅助认知训练对PSCI患者康复效果影响的随机对照试验,检索时限均为建库至2022年8月。由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用RevMan 5.3软件进行Meta分析。结果共纳入35项研究,包括1110例试验组患者,1095例对照组患者。Meta分析结果显示,计算机辅助认知训练组简易精神状况检查量表(MD=2.63,95%CI:2.13~3.14,P<0.001)、蒙特利尔认知评估量表(MD=2.90,95%CI:2.58~3.23,P<0.001)和洛文斯顿作业疗法认知评定量表(MD=8.11,95%CI:6.35~9.87,P<0.001)得分均高于对照组。结论计算机辅助认知训练对PSCI患者的认知功能有促进作用。 Objective:To systematically evaluate the effect of computer-assisted cognitive training on cognitive function of patients with cognitive impairment after stroke.Methods:Computer search of PubMed,Web of Science,CNKI,Wanfang and other databases were used to retrieve the randomized controlled trials of computer-assisted cognitive training for patients with cognitive impaired after stroke.The retrieval time was from the establishment of the database to August 2022.After two researchers independently screened the literature,the data were extracted and the bias risk included in the study was evaluated,and the meta-analysis was conducted with RevMan 5.3 software.Results:Totally 35 studies were included,including 1110 cases in the trial group and 1095 cases in the control group.The results of meta-analysis showed that the scores of the computer assisted cognitive training group,Montreal cognitive assessment(MD=2.90,95%CI:2.58~3.23,P<0.001)and Loewenstein occupational therapy cognitive assessment(MD=8.11,95%CI:6.35~9.87,P<0.001)were higher than those of the control group in the mini-mental status examination(MD=2.63,95%CI:2.13~3.14,P<0.001),Conclusion:The current data show that computer-assisted cognitive training can promote the cognitive function of stroke patients with cognitive impairment.
