基于“互联网+医联体”构建糖尿病足慢病管理新模式

Build a new management model for chronic diabetic foot disease based on"Internet+Medical Consortium"

目的:建立基于移动互联网技术与医联体模式的糖尿病足新型分级诊疗体系。方法:建立院内报告中心,实现医学数据统一存储;以数据交换服务总线ESB为核心技术支撑,打造医联体内医疗信息互联网互通平台;基于ETL+CDC完成全量数据和增量数据的集成,实现医疗信息采集、清洗、存储、加载和决策辅助;以患者主索引EMPI为基础,实现电子病历等数据整合。结果:采用移动互联网+医联体模式下糖尿病足溃疡分级诊疗方案后,患者就诊及时率及满意度显著提高,大截肢率显著降低,患者住院时间缩短,住院治疗费用降低。结论:互联网+医联体模式下糖尿病足病的分级诊疗方案可满足糖尿病足患者的就医需求,使优质医疗资源有效下沉,切实落实分级诊疗。

Objective To establish a new tiered medical diagnosis and treatment system for diabetic foot based on the combination of mobile Internet technology and Medical Consortium.Methods An in-hospital report center was established to realize the unified storage of medical data,and the Internet interoperability platform of medical information within the Medical Consortium was established with the core technology support of data ESB(Exchange Service Bus).To implement collection,cleaning,storage,loading and decision aids of medical information by integrating the full and incremental data based on ETL+CDC.Based on EMPI,electronic medical records and other data were integrated.Results After adopting the mobile Internet+Medical Consortium model of tiered diagnosis and treatment of diabetic foot ulcer,the rate of timely diagnosis and satisfaction of patients with diabetic foot ulcer was significantly improved,the rate of major amputation was significantly reduced,the length of hospitalization of patients was shortened,and the cost of hospitalization was reduced.Conclusion The tiered diagnosis and treatment model for diabetic foot disease under the mode of Internet+Medical Consortium can meet the medical needs of patients with diabetic foot disease,make high-quality medical resources available to the mass,and implement the tiered diagnosis and treatment effectively.
