摘要
目的 应用Meta分析评价维生素D辅助治疗儿童肺炎的临床效果。方法 计算机检索中国知网(CNKI)、维普网、中国生物医学文献服务系统、万方数据知识服务平台、PubMed、Embase、Web of Science、Cochrane数据库,检索时限均为建库至2022年4月,检索内容主要涉及维生素D辅助治疗儿童肺炎临床效果的随机对照研究。由两名研究者独立筛选提取文献,并对纳入文献进行质量评价,通过Revman5.4软件进行数据分析。结果 本次共纳入15篇文献,共2239例患儿。结果显示疾病复发率(RR=0.82, 95%CI:0.70~0.97),临床有效率(RR=1.16, 95%CI:1.11~1.22),住院时间(SMD=-0.65, 95%CI:-1.09~-1.20),热退时间(SMD=-0.62, 95%CI:-1.09~-0.14),气促喘息消失时间(SMD=-0.97, 95%CI:-1.49~-0.44),肺部啰音消失时间(SMD=-1.04, 95%CI:-1.19~-0.88)比较,差异均有统计学意义(P均<0.05);不良反应发生率(RR=1.12, 95%CI:0.83~1.50)和疾病死亡率(RR=1.44, 95%CI:0.43~4.78)比较,差异均无统计学意义(P均>0.05)。结论 维生素D辅助治疗儿童肺炎可以减少疾病复发率,增加临床治疗有效率,住院时间、热退时间、气促喘息时间和肺部啰音消失时间均可缩短,但不良反应发生率和死亡率不会降低。
Objective To evaluate the clinical effect of vitamin D in adjuvant treatment of pneumonia in children by Meta-analysis.Methods We searched CNKI,VIP,CBM,Wanfang data,PubMed,Embase,Web of Science and Cochrane from database inception until April 2022.The content of the search mainly included a randomised controlled study of the clinical impact of vitamin D in adjuvant treatment of pneumonia in children.Two researchers independently screened and extracted the literature,evaluated the quality of the included literature.Revman5.4software was used for statistical analysis.Results A total of 15 literatures were included,including 2239 children.The results showed the disease recurrence rate(RR=0.82,95%CI:0.70-0.97),clinical effective rate(RR=1.16,95%CI:1.11-1.22),hospitalization time(SMD=-0.65,95%CI:-1.09--1.20),fever time(SMD=-0.62,95%CI:-1.09--0.14),disappearance time of shortness of breath(SMD=-0.97,95%CI:-1.49--0.44),disappearance time of pulmonary rales(SMD=-1.04,95%CI:-1.19--0.88)between two groups were statistically significant(all P<0.05);adverse reaction rate(RR=1.12,95%CI:0.83-1.50);and mortality of children(RR=1.44,95%CI:0.43-4.78)between two groups were not statistically significant(all P>0.05).Conclusion Vitamin D adjuvant therapy in children with pneumonia can reduce the recurrence rate of the disease,increase the effective rate of clinical treatment,and shorten the hospitalization time,fever time,shortness of breath,and disappearance time of pulmonaryrales,but it might not reduce the adverse reaction rate and mortality among children.
