经皮无创穴位电刺激在治疗突发性聋中的应用

Application of transcutaneous non-invasive acupoint electrical stimulation in the treatment of sudden deafness

目的探讨经皮无创穴位电刺激在治疗突发性耳聋中的应用价值。方法选取90例单耳突发性聋患者按随机数字表法分成三组,药物+穴位电刺激组,药物+高压氧治疗组,药物治疗组。药物组予以激素、改善微循环联合营养神经药物,穴位电刺激组在药物治疗的基础上采用本课题组的专利产品进行穴位刺激。选取听宫穴+翳风穴、听宫穴+中渚穴、中渚穴+翳风穴三组穴位,每组穴位治疗15分钟,每天2次,连续治疗15d。药物+高压氧治疗组在药物治疗的基础上加用每日一次的高压氧治疗,疗程为15天。在15天疗程结束时比较三组疗效,同时比较突聋后的耳鸣改善情况。结果治疗15天结束时:药物+穴位电刺激组纯音平均听阈总有效率76.67%,治愈率40%,突聋后耳鸣治疗有效率83.33%;药物+高压氧组纯音平均听阈总有效率40.0%,治愈率16.67%,突聋后耳鸣治疗有效率52.63%;单纯药物组纯音平均听阈总有效率43.33%,治愈率3.33%,突聋后耳鸣治疗有效率45.45%。药物+穴位电刺激组纯音平均听阈的总有效率和治愈率显著大于其它二组(P<0.05),突聋后耳鸣治疗有效率同样显著优于另外二组(P<0.05)。结论无创穴位电刺激在突发性聋的治疗中具有良好的效果,值得进一步临床探索。

Objective To investigate the value of transcutaneous non-invasive acupoint electrical stimulation in the treatment of sudden deafness.Methods Ninety patients with sudden deafness in one ear who were hospitalized in the Department of Otolaryngology,Head and Neck Surgery,The First Affiliated Hospital of Anhui Medical University from August 1,2019,to December 31,2020,were selected and divided into three groups according to the random number table method,drug+acupoint electrical stimulation group,drug+hyperbaric oxygen treatment group,and control group(drug treatment group).The drug group was given hormones,improved microcirculation combined with neurotrophic drugs.The electrical stimulation group used our patented product for acupoint stimulation.Three groups of acupuncture points were selected,namely,the acupuncture point of the Tinggong+Yifeng,the acupuncture point of the Tinggong+Zhongzhu,and the acupuncture point of the Zhongzhu+Yifeng,each group was treated for 15 minutes,twice a day for 15 days.The drug+hyperbaric oxygen group added hyperbaric oxygen treatment once a day for 15 days.The efficacy of the three groups was compared at the end of the 15-days treatment.Results At the end of 15 days of treatment,the total effective rate of pure tone hearing threshold in the drug+acupoint electrical stimulation group was 76.67%,the cure rate was 40%,and the effective rate of post-sudden deafness tinnitus treatment was 83.33%.The total effective rate of pure tone hearing threshold in the drug+hyperbaric oxygen group was 40.0%,the cure rate was 16.67%,and the effective rate of post-sudden deafness tinnitus treatment was 52.63%.The total effective rate of audiology in the drug-only group was 43.33%,the cure rate was 3.33%and the effective rate of tinnitus after sudden deafness treatment was 45.45%.The total effective rate and cure rate of pure tone hearing threshold in the drug+acupoint electrical stimulation group were significantly greater than those in the other two groups(P<0.05),and the effective rate of tinnitus treatment after sudden deafness was also considerably better than those in the other two groups(P<0.05).Conclusion Non-invasive acupoint electrical stimulation has a good effect on treating sudden deafness and deserves further clinical exploration.
