基于Meta分析桥接网络药理学耳聋胶囊治疗突聋的疗效评价及作用机制

摘要 目的通过Meta分析对耳聋胶囊治疗突发性聋的临床疗效进行评价以及基于网络药理学对耳聋胶囊治疗突发性聋的作用机制进行研究。方法网络检索Web of Science、pubmed、维普、万方和中国期刊全文数据库文献(建库~2022年1月),收集耳聋胶囊联合常规西药治疗突发性聋随机对照试验的文献,并按照纳入和排除标准进行筛选,再利用RevMan5.4软件进行Meta分析。从TCMSP数据库、Uniprot数据库、Gene Cards数据库分别获取耳聋胶囊的有效化学成分及相对应的靶点基因和突发性聋的靶点基因。采用Cytoscape软件构建药物-有效成分-靶点交互网络;通过FunRich软件获取共有靶点;使用DAVID平台对筛选出的靶点信息进行GO生物功能和KEGG通路富集分析,并基于Cytoscape软件进行关键靶点的筛选。结果共5篇文献纳入研究,耳聋胶囊联合常规西药的临床有效率更高(P<0.00001),可以更好的改善血液低切粘度、高切粘度和血浆粘度(P<0.00001)。耳聋胶囊治疗突发性聋的关键靶点为TNF、PPARG、GJA1、IL-1β、IL-6。主要信号通里包括C型凝集素受体信号通路、IL-17信号通路、Toll样受体信号通路、AGE-RAGE信号通路、TNF信号通路、NOD样受体信号通路等。结论现有文献表明耳聋胶囊联合西药比单用西药治疗突发性聋有效性更高,改善血液流变学效果更好。耳聋胶囊通过多靶点和多通路治疗突发性聋,其作用机制可能与抗炎、抗氧化、调节免疫、改善循环等方面有关。 Objective The efficacy of Erlong Capsule in the treatment of sudden deafness was evaluated in this study.Based on Meta-analysis and network pharmacology,the potential mechanism of Erlong Capsule in sudden deafness treatment was investigated.Methods Web of Science,pubmed,VIP,Wanfang detabase and CNKI(since January 2022)were used to retrieve and screen the literature on sudden deafness treatment using Erlong Capsule.Randomized controlled trial of Erlong Capsule combined with conventional Western medicine in the treatment of sudden deafness were selected,and screened according to inclusion and exclusion criteria.Meta-analysis was performed for studies that using RevMan 5.4 software.Targets of Erlong Capsules were screened using the TCMSP and Uniprot.Targets of sudden deafness were screened using the GeneCards database.Key target screening was performed and Drug-Active Component-Target Interaction Network was constructed using the Cytoscape Software.Both targets,Erlong Capsule and sudden deafness,were screened using the FunRich.DAVID platform was used to analyze the GO and KEGG pathways on the common targets of sudden deafness and Erlong capsule.Results A total of 5 literatures were included in the study.The clinical effective rate of Erlong capsule combined with conventional western medicine was higher(P<0.00001),and it could better improve the blood low shear viscosity,high shear viscosity and plasma viscosity(P<0.00001).The key targets of Erlong capsule in the treatment of sudden deafness are TNF,PPARG,GJA1,IL-1βand IL-6.Major signaling pathways include C-type lectin receptor signaling pathway,IL-17 signaling pathway,Toll-like receptor signaling pathway,AGE-RAGE signaling pathway,TNF signaling pathway,Nod-like receptor signaling pathway,etc.Conclusion The existing literature shows that Erlong capsule combined with Western medicine is more effective than Western medicine alone in the treatment of sudden deafness and has better effect on improving hemorheology.Erlong capsule can treat sudden deafness through multi-target and multi-pathway,and its mechanism may be related to anti-inflammatory,antioxidant,immune regulation and circulation improvement.
