田道法教授基于“气虚染毒”学说治疗鼻咽癌临证经验总结

Professor Tian Dao-fa’s clinical experience in treating nasopharyngeal carcinoma based on the academic theory of“Contamination due to deficiency of Qi”

目的分析总结田道法教授处理鼻咽癌(颃颡岩)相关临床问题的临证经验,归纳其学术思想。方法回顾性分析田道法教授亲历之临床诊疗病案与学术研究历程,系统整理临证诊疗思维,总结其临床诊疗经验,归纳学术思想。结果在中医古文献研究及鼻咽癌高危人群中医体质类型临床流行病学调研基础上,通过系统总结临证诊疗经验,田道法教授创立了鼻咽癌的“气虚染毒”病机学说,据此确立了“益气解毒”治疗法则,应用于鼻咽癌高危人群的防护性治疗及晚期鼻咽癌的中医挽救性治疗、接受放化疗鼻咽癌患者的减毒增效、鼻咽癌患者疗后康复和复发转移防治等领域,弥补了当代主流治疗的缺陷和不足。结论依据“气虚染毒”学说认识鼻咽癌的中医病机,采用“益气解毒”法则指导鼻咽癌临床疑难问题的治疗,为解决鼻咽癌临床诊疗难题提供了新的视角。

Objective To induce Professor Tian Daofa’s academic ideology on the associated clinical issues with nasopharyngeal carcinoma(NPC,i.e.Hang Sang Yan in the term of TCM,)based on analyzing and summarizing his clinical experience in treating these subjects in this field.Methods Through a systematical retrospective analysis on the cases with NPC treated during the past years by Professor Tian himself and his series academic research in this field till now,Professor Tian’s academic ideology was induced via systematically organizing his clinical diagnosis and treatment thinking in the light of summing up clinical experience.Results On the basis of ancient literatures review and clinical epidemiological survey on the types of constitution,in terms of TCM,of population at high risk of NPC,Professor Tian formulated the theory of pathogenesis“Contamination due to deficiency of Qi”for the development of NPC and the treating principle of“Qi-boosting and toxin-resolving”for the treatment of such a condition via systematically summing up his own clinical experience in treating such cases.Then,put into clinical practice with the pathogenesis theory and treating principle were in such fields as protective treatment of population at high risk of NPC and salvage therapy for advanced NPC cases,toxicity reducing and efficacy enhancing treatment for NPC cases receiving chemoradiotherapy,and rehabilitation therapy for NPC cases following chemoradiotherapy and prophylaxis and treatment for NPC cases finished their routine chemoradiotherapy.In this way,remedied were the weaknesses and defects of currently used mainstream therapies for the treatment of NPC.Conclusions New perspective has been provided in the resolve of NPC-associated difficult problems in its diagnosis and treatment when the pathogenesis of NPC is recognized according to the theory of pathogenesis of NPC as“Contamination due to deficiency of Qi”and the treating principle of“Qi-boosting and toxin-resolving”,induced from the mainstream academic ideology of Prof.Tian,is taken to guide the treatment of NPC-associated clinical conditions,.
