摘要
目的探讨解整合素-金属蛋白酶33(ADAM-33)对发热的慢性阻塞性肺疾病急性加重期(AECOPD)患者分层治疗的指导价值。方法选取2020年1月至2022年4月该院伴发热的AECOPD患者98例为观察组,同期不伴发热的AECOPD患者49例为对照组。检测两组患者血清ADAM-33水平,分析铜绿假单胞菌感染的影响因素,绘制受试者工作特征曲线评估血清ADAM-33水平诊断铜绿假单胞菌感染的价值。结果观察组血清ADAM-33水平高于对照组(P<0.05),血清ADAM-33水平是铜绿假单胞菌感染的独立影响因素(P<0.05)。血清ADAM-33水平评估铜绿假单胞菌感染的曲线下面积为0.855,灵敏度为78.95%,特异度为79.75%。结论血清ADAM-33水平可预测AECOPD发热患者铜绿假单胞菌感染风险,有效指导临床进行分层治疗。
Objective To explore the guiding value of a disintegrin and metalloproteinase-33(ADAM-33)in stratified treatment of febrile patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 98 AECOPD patients with fever in our hospital from January 2020 to April 2022 were selected as the observation group,and 49 AECOPD patients without fever during the same period were selected as the control group.The serum ADAM-33 level of the two groups was detected,and the influencing factors of Pseudomonas aeruginosa infection were analyzed.Moreover,the value of serum ADAM-33 level in the diagnosis of Pseudomonas aeruginosa infection was evaluated by receiver operating characteristic curve.Results The serum ADAM-33 level in the observation group was higher than that in the control group(P<0.05).Serum ADAM-33 level was an independent influencing factor for Pseudomonas aeruginosa infection(P<0.05).The area under curve of serum ADAM-33 level for assessing Pseudomonas aeruginosa infection was 0.855,with sensitivity of 78.95%and specificity of 79.75%.Conclusion Serum ADAM-33 level could predict the risk of Pseudomonas aeruginosa infection in febrile patients with AECOPD,and effectively guide clinical stratified treatment.
