芒针围刺“股五针”治疗气滞血瘀型股骨头缺血性坏死的临床研究

Clinical Study of Surrounding Awn Needling‘Femoral Five-Needle’in Treating ONFH of Qi Stagnation and Blood Stasis

目的:探究芒针围刺“股五针”治疗气滞血瘀型早期股骨头缺血性坏死的临床疗效。方法:将符合纳入标准的60例气滞血瘀型早期股骨头缺血性坏死患者随机分为芒针围刺组(观察组)、体外冲击波组(对照组),各30例。对照组给予体外冲击波联合口服三七活骨丸治疗,观察组于对照组治疗基础上给予芒针围刺“股五针”治疗。两组疗程均为30 d。观察两组治疗前后VAS疼痛评分、血清总胆固醇、低密度脂蛋白胆固醇、甘油三酯、血浆黏度及血浆D-二聚体的变化,并根据Harris评分进行临床疗效评定。结果:观察组总有效率93.33%(28/30),对照组总有效率76.67%(23/30),观察组明显高于对照组,差异具有统计学意义(P<0.05);两组患者治疗后VAS疼痛评分、血清总胆固醇、低密度脂蛋白胆固醇、甘油三酯、血浆黏度及血浆D-二聚体与治疗前比较均明显改善,差异具有统计学意义(P<0.01),且观察组明显优于对照组,差异具有统计学意义(P<0.01)。结论:芒针围刺“股五针”治疗气滞血瘀型股骨头缺血性坏死能有效改善髋关节疼痛,对血脂、血液流动学有明显改善作用,可缓解坏死股骨头周围的血液供应,有利于髋关节功能的恢复。

Objective:To explore the clinical effect of surrounding awn needling‘femoral five-needle’in treatment of early osteonecrosis of femoral head(ONFH)with Qi stagnation and blood stasis.Methods:A total of 60 patients with early ONFH of Qi stagnation and blood stasis were randomly divided into the observation group and the control group,with 30 cases in each group.The control group was treated with extracorporeal shock wave combined with Sanqi Huogu Pill;on which basis,the observation group was also treated with surrounding awn needling‘femoral five-needle’.The treatment course of each group was 30 days.VAS pain score,serum total cholesterol,low density lipoprotein cholesterol,triglyceride,plasma viscosity and plasma D-dimer were observed before and after the treatment in the two groups,and the clinical efficacy was evaluated according to Harris score.Results:The total effective rate of the observation group was 93.33%(28/30),which was significantly higher than 76.67%(23/30)of the control group(P<0.05).VAS pain score,serum total cholesterol,low density lipoprotein cholesterol,triglyceride,plasma viscosity,plasma D-dimer were significantly improved after the treatment compared with those before the treatment in the two groups(P<0.01);and the improvements were more significant in the observation group than those in the control group(P<0.01).Conclusion:Surrounding awn needling‘femoral five-needle’can effectively improve the pain of the hip joint,blood lipids and hemodynamics,relieve the blood supply around the necrotic femoral head,and is conducive to the recovery of hip joint function in treatment of ONFH of Qi stagnation and blood stasis.
