局部亚低温联合血肿清除术治疗外伤性颅内血肿的效果观察

摘要 目的:分析局部亚低温联合血肿清除术治疗外伤性颅内血肿的效果。方法:选取2017年1月—2021年9月重庆市十三人民医院收治的60例外伤性颅内血肿患者作为研究对象,根据患者所接受的干预方式分为试验组与对照组,各30例。对照组给予保守内科治疗,试验组给予早期颅内微创穿刺联合局部亚低温治疗,比较两组干预前后斯堪的那维亚卒中量表(SSS)评分、头部血肿体积、干预后3个月临床预后情况,以及并发症发生情况。结果:干预前,两组SSS评分比较,差异无统计学意义(P>0.05);干预后7、14 d及3个月,试验组SSS分数高于对照组,差异有统计学意义(P<0.05)。干预前,两组头部血肿体积比较,差异无统计学意义(P>0.05);干预后3、7、14 d,试验组头部血肿体积小于对照组,差异有统计学意义(P<0.05)。干预后3个月,试验组活动能力分数60~100分及40~59分者多于对照组,差异有统计学意义(P<0.05)。结论:外伤性颅内血肿患者实施局部亚低温联合血肿清除术治疗效果显著,能改善患者脑出血量,提升身体活动能力,促进患者疾病转归,安全性强。 Objective:To analyze the effect of local mild hypothermia combined with hematoma removal in the treatment of traumatic intracerebral hematoma.Methods:A total of 60 patients with traumatic intracerebral hematoma admitted to the Thirteen People's Hospital of Chongqing from January 2017 to September 2021 were selected as the study subjects.According to intervention methods,the patients were divided into control group and experimental group,with 30 cases in each group.The control group received conservative medical treatment,and the experimental group received early intracranial minimally invasive puncture combined with local mild hypothermia.The Scandinavian Stroke Scale(SSS)score and brain hematoma volume before and after the intervention,clinical prognosis at 3 months post-intervention,and the occurrence of complications were compared between the two groups.Results:Before the intervention,there was no significant difference in SSS scores between the two groups(P>0.05).At 7 d,14 d and 3 months after the intervention,the SSS scores in the experimental group were higher than those in the control group,and the difference was statistically significant(P<0.05).Before the intervention,there was no significant difference in the volume of brain hematoma between the two groups(P>0.05).At 3 d,7 d and 14 d after the intervention,the volume of brain hematoma in the experimental group was smaller than that in the control group,and the difference was statistically significant(P<0.05).At 3 months after the intervention,the number of patients with the activity score of 60-100 points and 40-59 points in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:For patients with traumatic intracerebral hematoma,local mild hypothermia combined with hematoma removal has significant treatment effects.This method can improve the amount of intracerebral hemorrhage,improve the ability of body activity,promote the disease outcome of patients,with strong safety.
