手术时机选择在可手法复位的成人腹股沟嵌顿疝中的应用效果对照研究

Controlled Study on Application Effect of Surgical Timing Selection in Adult Incarcerated Inguinal Hernia with Manual Reduction

比较手术时机的不同选择对可手法复位的成人腹股沟嵌顿疝的应用效果。方法:选取2014年6月—2021年6月武警陕西省总队医院收治的成人嵌顿性腹股沟疝符合手法复位条件,复位成功的80例患者作为研究对象,根据随机数字表法分为观察组和对照组,各40例。观察组在复位成功后1周,对照组在复位成功后48 h分别行疝环充填式无张力修补术,术后均随访12个月。比较两组术后疼痛情况、并发症发生情况。结果:术后第1天,两组视觉模拟评分(VAS)比较,差异无统计学意义(P=0.052);术后第2、3天,观察组VAS评分低于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率低于对照组,差异有统计学意义(P=0.017)。结论:成人腹股沟嵌顿疝手法复位成功1周后手术能有效降低术后并发症发生率,有利于患者恢复。

Objective:To compare the application effect of different surgical timing selections in adult incarcerated inguinal hernia that can be reduced by manipulation.Methods:A total of 80 adult patients with incarcerated inguinal hernia admitted to Shaanxi Provincial Armed Police Corps Hospital from June 2014 to June 2021 were collected,all patients met the conditions of manual reduction and succeeded in reduction.They were divided into control group and observation group according to random number table method,with 40 cases in each group.The hernia ring filling tension-free repair was performed at 1 week after successful reduction in observation group and at 48 h after successful reduction in control group.After surgery,both groups were followed up for 12 months.Postoperative pain and postoperative complications between the two group were compared.Results:On postoperative day 1,there was no significant difference in visual analogue scale(VAS)score between the two groups(P=0.052).On postoperative days 2 and 3,the VAS score in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.017).Conclusion:Surgery at 1 week after successful manual reduction of adult incarcerated inguinal hernia can effectively reduce the incidence of postoperative complications,and is beneficial to the recovery of patient.
