卡孕栓联合垂体后叶素在剖宫产术中出血患者中的应用效果观察

Observations on Application Effect of Carboprost Methylate Suppositories Combined with Pituitrin in Patients with Hemorrhage During Cesarean Section

目的:探讨卡孕栓联合垂体后叶素在剖宫产术中出血患者中的应用效果。方法:选取2020年6月—2022年6月金乡县人民医院收治的60例剖宫产术中出血患者作为研究对象,采用随机数字表法分为观察组与对照组,各30例。对照组给予垂体后叶素治疗,观察组在对照组基础上给予卡孕栓治疗。比较两组围术期指标、凝血指标。结果:观察组术中、术后1~2 h、术后2~24 h出血量及输血量均少于对照组,住院时间、宫缩持续时间短于对照组,差异有统计学意义(P=0.000)。术前,两组D-二聚体(D-D)、纤维蛋白原(FIB)、凝血酶时间(TT)、凝血酶原时间(PT)、部分凝血活酶时间(APTT)比较,差异无统计学意义(P>0.05);术后1 d,观察组D-D水平高于对照组,FIB水平低于对照组,APTT短于对照组,差异有统计学意义(P<0.05);两组TT、PT比较,差异无统计学意义(P>0.05)。结论:卡孕栓联合垂体后叶素在剖宫产术中出血患者中的应用效果显著,可减少患者术中、术后出血量,改善患者凝血指标。

Objective:To investigate the application effect of carboprost methylate suppositories combined with pituitrin in the patients with hemorrhage during cesarean section.Methods:From June 2020 to June 2022,60 patients with hemorrhage during cesarean section admitted to Jinxiang County People's Hospital were selected as the study subjects.They were divided into the control group and the observation group according to random number table method,with 30 patients in each group.The control group received pituitrin treatment,and the observation group received carboprost methylate suppositories treatment on the basis of control group.Perioperative indicators and coagulation indicators of the two groups were compared.Results:The amount of bleeding and blood transfusion in the observation group during operation,at 1-2 h and 2-24 h after operation was lower than that in the control group,and the duration of hospitalization and uterine contraction in the observation group was shorter than that in the control group,and the difference was statistically significant(P=0.000).Before surgery,there was no significant difference between the two groups in D-dimer(D-D),fibrinogen(FIB),thrombin time(TT),prothrombin time(PT),and partial thromboplastin time(APTT)(P>0.05).On 1 d after surgery,the D-D level in the observation group was higher than that in the control group,the FIB level in the observation group was lower than that in the control group,and the APTT in the observation group was shorter than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in TT and PT between the two groups(P>0.05).Conclusion:The application effect of carboprost methylate suppositories combined with pituitrin in the patients with hemorrhage during cesarean section is significant,it can reduce the amount of intraoperative blood loss and postoperative bleeding,and improve the blood coagulation index of patients.
