椎旁肌退变与退变性腰椎滑脱的相关性分析

摘要 目的:探讨椎旁肌退变与退变性腰椎滑脱的相关性。方法:选取2021年1—11月烟台市莱阳中心医院脊柱外科门诊收治的85例慢性腰痛患者作为研究对象,根据影像学诊断结果,将慢性腰背痛患者纳入对照组,将腰椎滑脱患者纳入观察组。应用ImageJ软件测量两组患者磁共振T2像上L4,L5中间水平面的竖脊肌、多裂肌、腰大肌横截面面积。结果:观察组L4,L5平面双侧多裂肌、竖脊肌、腰大肌横截面积均小于对照组,差异有统计学意义(P<0.05)。观察组L4,L5平面左侧竖脊肌与多裂肌、右侧竖脊肌与多裂肌、总的椎旁肌与多裂肌横截面积和均小于对照组,差异有统计学意义(P<0.05)。结论:椎旁肌退变可能导致退变性腰椎滑脱发生。 Objective:To investigate the correlation between paravertebral muscle degeneration and degenerative lumbar spondylolisthesis.Methods:A total of 85 patients with chronic low back pain admitted to Spine Surgery Clinic of Yantai Laiyang Central Hospital from January 2021 to November 2021 were selected as the study subjects.Patients with chronic low back pain were included in the control group and patients with lumbar spondylolisthesis were included in the observation group.ImageJ software was used to measure the cross-sectional areas of erector spinae muscle,multifidus muscle and psoas major muscle on L4 and L5 intermediate horizontal level by MRI T2 image of patients in the two groups.Results:The cross-sectional areas of bilateral multifidus muscles,erector spinae muscles,and psoas major muscles on L4 and L5 level in the observation group were smaller than those in the control group,and the difference was statistically significant(P<0.05).The cross-sectional areas of the left erector spinae muscle and multifidus muscle,the right erector spinae muscle and multifidus muscle,and the total paravertebral muscle and multifidus muscle in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Degeneration of paravertebral muscles may lead to degenerative lumbar spondylolisthesis.
