俯卧位悬吊复位法联合单侧穿刺椎体后凸成形术治疗Ⅰ,Ⅱ期Kümmell病临床观察

Clinical observation of prone suspension reduction method combined with unilateral puncture kyphoplasty for Kimmell disease in stageⅠandⅡ

目的观察俯卧位悬吊复位法联合单侧穿刺椎体后凸成形术治疗Ⅰ,Ⅱ期Kümmell病的临床疗效。方法随机选取2020年~2021年行手术治疗的Ⅰ,Ⅱ期Kümmell病患者50例,根据治疗方式不同分为观察组和对照组,每组25例。观察组采用俯卧位悬吊复位法联合单侧穿刺椎体后凸成形术治疗,对照组仅采用单侧穿刺椎体后凸成形术治疗。记录两组术前及各观察节点的视觉模拟量表(VAS)评分、责任椎体前壁高度(VH)、Cobb角变化。结果与术前比较,两组术后1d、12周、24周的VAS评分降低,VH显著增高,Cobb角降低,差异有统计学意义(P<0.05);两组间比较,治疗组术后各观察节点VAS评分低于对照组(P<0.05),VH高于对照组(P<0.05),Cobb角低于对照组(P<0.05)。结论俯卧位悬吊复位法联合单侧穿刺椎体后凸成形术治疗Ⅰ,Ⅱ期Kümmell病对改善疼痛症状,恢复病椎高度,改善后凸畸形,减少创伤等方面效果明显。

Objective To observe the clinical effect of prone suspension reduction method combined with unilateral puncture kyphoplasty forⅠ,Ⅱstage Kimmell disease.Methods From October 2020 to October 2021,a total of 50 patients with Kimmell disease who underwent surgical treatment were randomly selected and divided into observation group and control group according to different treatment methods,with 25 cases in each group.The observation group was treated by prone position suspension and reduction method combined with unilateral puncture kyphoplasty,while the control group was treated with unilateral puncture kyphoplasty.The visual analog scale(VAS)score,anterior wall height(VH),and cobb angle were recorded.Resultss Compared with the preoperative results,VAS score decreased at 1 day,12 weeks and 24 weeks,VH was significantly increased,and the cobb angle decreased,statistically significant(P<0.05);The VAS score of each observation node in the treatment group was lower than those in the control group(P<0.05).VH in the treatment group was higher than that in the control group(P<0.05).The cobb angle in the treatment group was lower than that in the control group(P<0.05).Conclusion Prone suspension reduction method combined with unilateral puncture kyphoplastyⅠ,ⅡKimmell disease can improve the effect of pain symptoms,restore the vertebral height,improve kyphosis deformity,reduce trauma and other aspects.
