The Progress and Effect of the Public Hospital Salary System Reform Oriented by Increasing the Value of Knowledge in China

摘要 目的了解和分析以增加知识价值为导向的我国公立医院薪酬制度改革进展与成效。方法基于主题内容分析法和描述性统计分析方法归纳和评价我国各地区公立医院薪酬制度改革政策体系特点及其成效。结果在改革中,各地区主要以完善岗位绩效工资制度为主,逐步提高固定薪酬占比,薪酬支出主要来源于业务收入,不少地区推行院长目标年薪制,各地区公立医院人均工资水平一般设定为当地事业单位人均工资水平的2~4倍。整体上看,近几年全国公立医院人均工资水平已达到社会平均工资的1.8~1.9倍,公立医院人员经费支出占业务支出的比例2021年达到41.1%。结论我国公立医院薪酬制度改革取得了阶段性成效,公立医院负责人薪酬激励约束机制基本建成,公立医院人均工资水平、人员支出占业务支出比例不断提高,但是医务人员薪酬支付方式、薪酬结构和经费来源仍需要进一步优化和改进。 Objective To analyze and evaluate the progress and effect of the salary system reform of public hospitals China,which is oriented to increase the value of knowledge. Methods Based on the thematic content and descriptive statistical analysis methods,the policy characteristics and effect of the public hospital salary system reform in different regions of China were summarized and evaluate. Results In the process of reform,various regions mainly focus on improving the post-performance salary system,gradually increasing the proportion of fixed salary,the salary expenditure still mainly comes from service income,and many regions implement the hospital presidents' yearly salary system,the per capita wage level of public hospitals in various regions is generally controlled within 2~4 times of the per capita wage level of local(other) public institutions. On the whole,in recent years,the per capita wage of public hospitals across the country has reached 1.8~1.9 times of the average social wage. In 2021,the proportion of public hospital personnel expenditure in business expenditure reached 41.1%. Conclusion The reform of the remuneration system in public hospitals in China has achieved phased results. The incentive and restraint mechanism for the public hospital presidents' remuneration has been basically completed. The per capita wage level and the proportion of personnel expenditure in business expenditure in public hospitals have been continuously increasing. However,the way of payment,the structure of remuneration and the source of funds for medical personnel still need to be further optimized and improved.
