超声介入胆囊造瘘术在老年急性胆囊炎治疗中的应用价值

Application value of ultrasound intervention cholecystostomy in the treatment of elderly patients with acute cholecystitis

目的分析超声介入胆囊造瘘术在老年急性胆囊炎治疗中的应用价值。方法45例老年急性胆囊炎患者,使用随机分配法分为实验组(23例)和参照组(22例)。实验组实施超声介入胆囊造瘘术治疗,参照组实施传统胆囊造瘘术治疗。比较两组手术指标及并发症发生情况。结果实验组胆囊炎症控制时间为(1.28±0.53)d,与参照组的(1.39±0.41)d比较,差异无统计学意义(P>0.05);实验组住院时间(8.27±1.83)d短于参照组的(13.26±2.47)d,出血量(68.36±15.92)ml少于参照组的(126.34±24.91)ml,术后疼痛评分(1.72±0.43)分低于参照组的(2.95±0.67)分,差异有统计学意义(P<0.05)。实验组并发症总发生率为13.04%,明显低于参照组的40.91%,差异有统计学意义(P<0.05)。结论老年急性胆囊炎患者使用超声介入胆囊造瘘术治疗效果更好,可减少并发症的发生,有利于患者身体康复。

Objective To analyze the application value of ultrasound intervention cholecystostomy in the treatment of elderly patients with acute cholecystitis.Methods A total of 45 elderly patients with acute cholecystitis were randomly divided into experimental group(23 cases)and reference group(22 cases).The experimental group was treated with ultrasound intervention cholecystostomy,and the reference group was treated with conventional cholecystostomy.The surgical indexes and occurrence of complications were compared between the two groups.Results The control time of gallbladder inflammation in the experimental group was(1.28±0.53)d,and the difference was not statistically significant when compared with(1.39±0.41)d in the reference group(P>0.05).The length of hospital stay of(8.27±1.83)d in the experimental group was shorter than that of(13.26±2.47)d in the reference group;the amount of blood loss of(68.36±15.92)ml in the experimental group was less than that of(126.34±24.91)ml in the reference group;the score of postoperative pain of(1.72±0.43)points in the experimental group was lower than that of(2.95±0.67)points in the reference group;the differences were statistically significant(P<0.05).The total complication rate of the experimental group was 13.04%,which was significantly lower than that of 40.91%of the reference group,and the difference was statistically significant(P<0.05).Conclusion Ultrasound intervention cholecystostomy has better therapeutic effect in the treatment of elderly patients with acute cholecystitis,which can reduce the occurrence of complications,and is conducive to the physical rehabilitation of patients.
