中药制剂在某医院心血管疾病中的不合理用药情况

Non-rational Use of Traditional Chinese Medicine Preparations in Cardiovascular Diseases in a Hospital

目的 调查中药制剂在驻马店市中心医院心血管疾病中合理应用情况,以为临床合理、规范用药提供依据。方法 收集驻马店市中心医院心血管内科于2021年1月—12月收治的101例使用中药制剂治疗的患者病历,对病历中年龄、性别、用药种类、剂量、病情等有效信息进行统计,并参照用药标准,评价中药制剂在心血管疾病中的用药合理性,分析影响合理用药的相关因素。结果 101例病历中疾病类型主要为66例冠心病,占比65.35%(66/101);28例心肌梗死,占比27.72%(28/101);7例心律失常,占比6.93%(7/101)。主要剂型为丸剂、胶囊型、片剂型、颗粒型。其中用药频率最高的8种中药分别为心脑宁胶囊、通心络胶囊、地奥心血康胶囊、血滞通胶囊、丹七片、复方丹参片、稳心颗粒、冠心丹参丸。存在不合理用药情况主要包括用药剂量不宜、用药配伍不宜、药不对症型、未明确用法、单一药物长期服用,其中以用药剂量不宜、用药配伍不宜占比最高。结论 在临床应用中应着重强调用药合理性,重视用药剂量不宜、用药配伍不宜、药不对症型、未明确用法、单一药物长期服用等问题。

Objective To investigate the rational use of traditional Chinese medicine preparations in cardiovascular diseases in Zhumadian Central Hospital,so as to provide a basis for clinical rational and standardized use of drugs.Methods The medical records of 101 patients treated with traditional Chinese medicine from January to December 2021 in the Department of Cardiovascular Medicine of Zhumadian Central Hospital were collected,the effective information such as age,sex,type of medication,dosage and condition in the medical records were counted,and the rationality of the use of traditional Chinese medicine in cardiovascular diseases was evaluated by referring to the drug use standards,and the relevant factors affecting the rational use of drugs were analyzed.Results Among the 101 cases of medical records,the disease type was 66 cases of coronary heart disease,accounting for 65.35%(66/101).Twenty-eight cases of myocardial infarction,accounting for 27.72%(28/101).There were 7 cases of arrhythmia,accounting for 6.93%(7/101).The main dosage forms of traditional Chinese medicine preparations could be divided into capsule,tablet,granule and pill,among which the eight most frequently used traditional Chinese medicine preparations were Xinnaoning capsule,Tongxinluo capsule,Di’ao Xinxuekang capsule,Xuezhitong capsule,Danqi tablet,Compound Danshen tablet,Wenxin granule and Guanxin Danshen pill.Irrational drug use mainly included inappropriate dosage,inappropriate drug compatibility,drug mismatch syndrome,unclear usage,long-term use of a single drug,among which inappropriate dosage and inappropriate drug compatibility accounted for the highest proportion.Conclusion In clinical application,we should emphasize the rationality of drug use,pay attention to the inappropriate dosage,inappropriate drug compatibility,wrong type of drug,unclear use,long-term use of single drug and other problems.
