多学科模式下个体化运动康复护理在人工心脏永久性起搏器植入术后的应用效果

Application Effcet of Individualized Exercise Rehabilitation Nursing Under Multi-disciplinary Mode After Artificial Cardiac Permanent Pacemaker Implantation

目的 分析多学科模式下个体化运动康复护理在人工心脏永久性起搏器(PPM)植入术后的应用效果。方法 将2020年5月至2021年5月商丘市第一人民医院收治的35例PPM植入术后接受常规护理的患者纳入对照组,另将2021年6月至2022年6月35例PPM植入术后接受多学科模式下个体化运动康复护理的患者纳入观察组。两组患者连续干预至出院,比较两组患者术后恢复情况、护理前后心功能、心理状况、肩关节功能及护理后生活质量。结果 护理后,观察组术后下床活动、住院时间短于对照组,6分钟步行试验(6MWT)距离长于对照组(P<0.05);护理后,两组左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、心率(HR)高于护理前,观察组高于对照组(P<0.05);护理后,两组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分低于护理前,观察组低于对照组(P<0.05);护理后,两组日常生活能力、疼痛、肩关节活动功能及总分高于护理前,观察组高于对照组(P<0.05);观察组术后躯体功能、心理功能、社会功能、一般认知、生活质量总分高于对照组(P<0.05)。结论 多学科模式下个体化运动康复护理能够有效缓解PPM植入术患者心理压力,提高生活质量,改善心功能及肩关节功能,促进术后康复。

Objective To analyze the application effcet of individualized exercise rehabilitation nursing care under multi-disciplinary mode after implantation of artificial cardiac permanent pacemaker(PPM).Methods A total of 35 patients who received routine care after PPM implantation in the First People’s Hospital of Shangqiu from May 2020 to May 2021 were included in the control group,while another 35 patients who received individualized exercise rehabilitation care under the multidisciplinary model after PPM implantation from June 2021 to June 2022 were included in the observation group.Patients in the two groups received continuous intervention until they were discharged from hospital.The recovery after operation,cardiac function,psychological status,shoulder function and quality of life after care of patients in the two groups were compared.Results After nursing,the observation group had shorter ambulation and hospitalization time than the control group and six-minute walking test(6MWT)distance was longer than the control group(P<0.05).After care,left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),heart rate(HR)of the two groups were higher than those before care,and those in the observation group were higher than those in the control group(P<0.05).After nursing,the self-rating anxiety scale(SAS),self-rating depression scale(SDS)scores of the two groups were lower than those before nursing,and the scores in the observation group were lower than those in the control group(P<0.05).After nursing,the daily living ability,pain,shoulder joint activity function and total score of the two groups were higher than those before nursing,and the observation group was higher than the control group(P<0.05).The total scores of postoperative physical function,psychological function,social function,general cognition and quality of life in the observation group were higher than those in the control group(P<0.05).Conclusion Individualized exercise rehabilitation nursing under multi-disciplinary mode can effectively alleviate the psychological pressure of patients with PPM implantation,improve the quality of life,cardiac function and shoulder function,and promote postoperative rehabilitation.
