GCS评分、CT评分联合血清CRP水平对颅脑损伤患者损伤程度及预后的应用价值 被引量:1

Application value of GCS score,CT score combined with serum CRP level in the degree of injury and prognosis of patients with traumatic brain injury

摘要 目的探讨格拉斯哥昏迷评分(GCS)、计算机断层扫描评分(CT)联合血清C反应蛋白(CRP)水平对评价颅脑损伤(TBI)患者损伤程度及预后的应用价值。方法选择2019年3月至2021年9月莲花县人民医院收治的TBI患者98例,所有患者均按照GCS评分进行病情程度分级,并计算CT评分,检测CRP水平。比较不同病情程度、不同预后患者GCS评分、CRP水平与CT评分的差异,分析患者三项指标与病情程度、疾病预后的相关性。结果三组患者GCS评分相比,重度组<中度组<轻度组;三组CRP水平、CT评分相比,重度组>中度组>轻度组,差异有统计学意义(P<0.001);预后良好组GCS评分高于预后不良组,CRP水平、CT评分均低于预后不良组,差异有统计学意义(P<0.001)。经Pearson相关性分析显示,GCS评分与病情程度呈负相关(r=-0.459,P<0.05),CRP、CT评分与病情程度呈正相关(r值分别为0.361和0.245,P<0.05);GCS评分与预后呈正相关(r=0.538,P<0.05),CRP、CT评分与预后呈负相关(r=-0.546、-0.396,均P<0.05)。结论GCS、CT评分、CRP水平与TBI患者损伤程度及预后均存在密切关系,可用于临床早期诊断、疗效评估及预后判断。 Objective To investigate the application value of Glasgow coma scale(GCS),computed tomography(CT)score combined with serum C-reactive protein(CRP)level in the degree of injury and prognosis of patients with traumatic brain injury(TBI).Methods 98 patients with TBI admitted to Lianhua People's Hospital from March 2019 to September 2021 were selected.All patients were graded according to GCS scores,CT scores were calculated,and CRP levels were detected.And then,the GCS scores,CRP levels and CT scores of patients with different disease severity and prognosis were compared,and correlation between above-mentioned three indicators of patients and disease severity and disease prognosis was analyzed.Results Comparison of the GCS scores of three groups showed severe group<moderate group<mild group,and the comparison of CRP levels and CT scores of the three groups showed severe group>moderate group>mild group,and differences were statistically significant(P<0.001).The GCS score of the good prognosis group was higher than that of the poor prognosis group,and the CRP level and CT score were lower than those of the poor prognosis group,and the differences were statistically significant(P<0.001).Pearson correlation analysis showed that GCS score was negatively correlated with the severity of disease(r=-0.459,P<0.05),while CRP and CT score were positively correlated with the severity of disease(r=0.361,0.245,P<0.05).GCS score was positively correlated with prognosis(r=0.538,P<0.05),while CRP and CT score were negatively correlated with prognosis(r=-0.546,-0.396,all P<0.05).Conclusion GCS score,CT score and CRP level are closely related to the degree of injury and prognosis of TBI patients,which can be used for early clinical diagnosis,therapeutic effect evaluation and prognosis judgment.
