丹参多酚酸盐注射液对AECOPD患者肺功能、凝血及血气分析指标的影响

Effects of Salvianolate Injection on pulmonary function,blood coagulation and blood gas analysis indexes in AECOPD patients

目的观察丹参多酚酸盐注射液对慢性阻塞性肺疾病急性加重期(AECOPD)患者肺功能、凝血及血气分析指标的影响。方法将我院AECOPD患者98例随机分为对照组与观察组各49例。对照组采用常规西医治疗,观察组在对照组治疗基础上采用丹参多酚酸盐注射液治疗。比较两组治疗后临床疗效,治疗前后肺功能、凝血及血气分析水平,观察治疗期间不良反应发生情况。结果治疗后,观察组的总有效率高于对照组(P<0.05)观察组1秒钟用力呼气容积(FEV1)、最大呼吸流量(PEF)、1秒用力呼气率(FEV1/FVC%)水平与对照组比较无明显差异(P>0.05)观察组纤维蛋白原(FIB)、D-二聚体(D-dimer)水平低于对照组,凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)均高于对照组(P<0.05);观察组动脉血氧分压(PaO_(2))、pH水平高于对照组动脉血二氧化碳(PaCO_(2))低于对照组(P<0.05)治疗期间,两组均未出现明显不良反应。结论丹参多酚酸盐注射液治疗AECOPD患者能有效改善患者临床症状,改善其凝血功能,提高血液氧含量,降低CO_(2)潴留,有效保护患者肺部功能。

Objective To observe the effects of Salvianolate Injection on pulmonary function,blood coagulation and blood gas analysis indexes in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 98 AECOPD patients were randomly divided into control group and observation group,49 cases in each group.Control group was given routine western medicine,while observation group was additionally treated with Salvianolate Injection.The clinical curative effect after treatment,pulmonary function,blood coagulation and levels of blood gas analysis before and after treatment were compared between the two groups.The occurrence of adverse reactions during treatment was observed.Results After treatment,the total response rate of observation group was higher than that of control group(P<0.05);there was no significant correlation between FEV1,PEF and FEV1/FVC%in observation group and control group(P>0.05);the levels of fibrinogen(FIB)and D-dimer(D-dimer)were lower than those in control group,prothrombin time(PT)and activated partial thromboplastin time(APTT)were higher than those in control group(P<0.05);the levels of partial pressure of oxygen(PaO_(2))and pH in observation group were higher than those in control group,while partial pressure of carbon dioxide(PaCO_(2))was lower than that in control group(P<0.05).During treatment,there were no significant adverse reactions in either group.Conclusion Salvianolate Injection can effectively improve clinical symptoms and coagulation function of AECOPD patients,increase content of blood oxygen,reduce CO_(2)retention,and effectively protect lung function.
