CD147/MMP-9通路与2型糖尿病合并高血压患者发生颈动脉粥样硬化的关系

Relationship between CD147/MMP-9 Pathway and Carotid Atherosclerosis in Patients with Type 2 Diabetes Mellitus and Hypertension

目的探究2型糖尿病(T2DM)合并高血压患者中CD147/基质金属蛋白酶-9(MMP-9)通路与颈动脉粥样硬化(CAS)发生的关系。方法选取T2DM合并高血压患者100例,根据颈动脉内中膜厚度(IMT)将其分为2组:观察组(存在CAS)28例、对照组(不存在CAS)72例。比较2组血清CD147、MMP-9水平;多因素Logistic回归分析T2DM合并高血压患者CAS发生的影响因素;受试者工作特征(ROC)曲线下面积(AUC)评价血清CD147、MMP-9对T2DM合并高血压患者CAS的诊断价值;Pearson相关分析血清CD147、MMP-9与CAS程度(IMT、斑块指数)的相关性。结果与对照组比较,观察组血清CD147、MMP-9水平更高(均P<0.05);血清CD147、MMP-9均为T2DM合并高血压患者CAS发生的危险因素(均P<0.05);血清CD147、MMP-9联合诊断T2DM合并高血压患者CAS的AUC值为0.907,明显高于单一指标检测;T2DM合并高血压患者血清CD147、MMP-9与IMT、斑块指数呈正相关(均P<0.05)。结论CD147/MMP-9通路参与T2DM合并高血压患者CAS的发生,是防治T2DM合并高血压患者CAS发生的靶点。

Objective To investigate the relationship between CD147/matrix metalloproteinase-9(MMP-9)pathway and carotid atherosclerosis(CAS)in type 2 diabetes mellitus(T2DM)patients with hypertension.Methods A total of 100 patients with T2DM and hypertension were divided into two groups according to the carotid artery intima-media thickness(IMT):observation group(28 patients with CAS)and control group(72 patients without CAS).Serum CD147 and MMP-9 levels were compared between the two groups.The factors affecting the occurrence of CAS in T2DM patients with hypertension were analyzed using multivariate logistic regression analysis.The area under receiver operating characteristic curve(AUC)was used to evaluate the diagnostic values of CD147 and MMP-9 in T2DM patients with hypertension.The correlation between serum CD147 and MMP-9 levels and CAS degree(IMT and plaque index)was analyzed using Pearson correlation analysis.Results Serum CD147 and MMP-9 levels in the observation group were higher than those in the control group(P<0.05).Both CD147 and MMP-9 were risk factors for the occurrence of CAS in T2DM patients with hypertension(P<0.05).The AUC value of combined CD147 and MMP-9 detection in the diagnosis CAS was 0.907,significantly higher than that of single determination.Serum CD147 and MMP-9 levels were positively correlated with IMT and plaque index in patients with T2DM and hypertension(P<0.05).Conclusion CD147/MMP-9 pathway is involved in the occurrence of CAS,and can be used as a target for the prevention and treatment of CAS in T2DM patients with hypertension.
