肺癌后抑郁障碍相关因素分析及解郁汤干预治疗研究

Analysis of Related Factors of Depression after Lung Cancer and the Intervention Effect of Jieyu Decoction

目的:探究肺癌抑郁障碍的影响因素,观察解郁汤治疗肺癌伴抑郁障碍的临床疗效,并分析内在机制。方法:将80例肺癌伴抑郁障碍患者,按照抑郁程度分为2组,分析抑郁障碍的影响因素。后将肺癌伴轻度抑郁障碍患者随机分为观察组和对照组,对照组采用常规治疗,观察组在常规治疗基础上给予解郁汤口服,在治疗2个月后观察HAMD、SDS评分,检测CD3^(+)及5-HT水平,并探究肺癌后抑郁障碍的独立危险因素。结果:病程、受教育年限及肿瘤分期对肺癌抑郁障碍有一定影响,观察组治疗2个月后HAMD、SDS评分以及CD3^(+)、5-HT水平较对照组差异明显(P<0.01),并且在时间和治疗方式上存在交互作用,CD3^(+)水平及KPS评分是肺癌后抑郁障碍的独立危险因素。结论:解郁汤能够显著改善患者抑郁症状,有可能与增强免疫或调节5-HT水平有关。

Objective:Objective to explore the influencing factors of depression in lung cancer,observe the clinical efficacy of Jieyu Decoction in the treatment of lung cancer with depression,and analyze the internal mechanism.Methods:80 cases of lung cancer patients with depression were divided into two groups according to the degree of depression.The patients in the control group were given conventional treatment,while the patients in the observation group were given Jieyu Decoction on the basis of conventional treatment.After 2 months of treatment,the HAMD and SDS scores were observed,and the levels of CD3^(+) and 5-HT were detected.Results:The course of disease,years of education and tumor stage have certain influence on depressive disorder of lung cancer.There were significant differences in HAMD,SDS scores,CD3^(+) and 5-HT levels in the observation group after 2 months of treatment compared with the control group(P<0.01),and there were interactions in time and treatment methods.Conclusion:Jieyu Decoction can significantly improve the depressive symptoms of patients,which may be related to enhancing immunity or regulating the level of 5-HT.
