不同鼻中隔偏曲矫正术治疗外伤性鼻中隔偏曲的效果对比

Comparison on the Curative Effect of Different Correction Surgery on Traumatic Nasal Septum Deviation

目的:观察鼻内窥镜下鼻中隔偏曲矫正术同期歪鼻整形与鼻内镜下二线减张法鼻中隔偏曲矫正术治疗外伤性鼻中隔偏曲的效果。方法:回顾性选取2019年10月-2021年12月在笔者医院因鼻外伤导致鼻中隔偏曲的98例行鼻中隔偏曲矫正术治疗的成年患者为研究对象,根据不同手术方法分为A组(50例,行鼻内窥镜下鼻中隔偏曲矫正术同期歪鼻整形)和B组(48例,鼻内窥镜下二线减张法鼻中隔偏曲矫正术)。所有患者术后6个月随访,观察两组患者的临床疗效,采用鼻整形结局评估量表(Rhinoplasty outcome evaluation,ROE)评估两组患者生存质量,采用视觉模拟评分法(Visual analogue scale,VAS)评估两组患者鼻部整形满意程度。比较两组患者术后并发症发生情况。结果:98例患者均顺利完成手术,术后6个月复查,A组临床治疗总有效率为96.00%(48/50)与B组的87.50%(42/48)比较,差异无统计学意义(P>0.05)。两组患者术后复查ROE评分均升高、VAS评分均降低(P<0.05),且A组术后3个月的ROE评分高于B组(P<0.05)。两组术后并发症发生率无统计学意义(P>0.05)。结论:鼻内窥镜下鼻中隔偏曲矫正术同期歪鼻整形与鼻内窥镜下二线减张法鼻中隔偏曲矫正术治疗外伤性鼻中隔偏曲均有显著效果,但前者更利于改善患者术后生存质量。

Objective To observe the curative effect of nasal endoscopic correction surgery combined with crooked nasal plastics and nasal endoscopic double-line tension relaxing on traumatic nasal septum deviation.Methods A total of 98 adult patients with nasal septum deviation due to nasal trauma who underwent correction surgery in the hospital were retrospectively enrolled as the research objects from October 2019 to December 2021.According to different surgical methods,they were divided into group A(50 cases,nasal endoscopic correction surgery combined with crooked nasal plastics)and group B(nasal endoscopic double-line tension relaxing).All were followed up for 6 months after surgery to observe clinical curative effect.The quality of life was evaluated by rhinoplasty outcome evaluation(ROE).The satisfaction with nasal plastics was evaluated by visual analog scale(VAS).The occurrence of postoperative complications was compared between the two groups.Results All the 98 patients successfully completed the surgery and underwent re-examinations at 6 months after surgery.There was no significant difference in total clinical response rate between group A and group B[96.00%(48/50)vs 87.50%(42/48)](P>0.05).After surgery,ROE scores were increased,VAS scores were decreased in both groups(P<0.05).At 3 months after surgery,ROE scores in group A were higher than those in group B(P<0.05).The difference in incidence of postoperative complications between the two groups was not statistically significant(P>0.05).Conclusion The curative effect of both nasal endoscopic correction surgery combined with crooked nasal plastics and nasal endoscopic double-line tension relaxing is significant on traumatic nasal septum deviation.However,the former is more beneficial to improve postoperative quality of life in patients.
