外科手术联合牵引矫治30例弯曲上颌年轻恒中切牙疗效分析

Clinical Analysis of 30 Cases of Dilaceration of Maxillary Immature Permanent Central Incisors Treated by Surgical Exposure and Orthodontic Treatment

目的:观察外科手术联合牵引矫治弯曲阻生年轻恒前牙的疗效。方法:收集自2019年以来笔者医院收治的30例弯曲阻生年轻恒前牙病例,患儿年龄7~10岁,运用外科开窗联合牵引方法,将弯曲阻生年轻恒前牙恢复至正常牙列位置。患牙为观察组,对侧同名牙作为正常对照组,在双侧中切牙均受损的病例中,以同颌侧切牙为对照组,术后随访24个月。CBCT测量治疗后观察组和对照组的唇侧牙槽骨厚度、牙冠高度和牙根长度,评估预后效果。结果:29例患者共30颗患牙,其中男15例,女14例,失访3例,平均年龄(7.98±0.63)岁,成功率96.3%。利用CBCT测量分析结果,治疗后,唇侧牙槽骨厚度小于对照组,且牙根长度低于对照组,差异均有统计学意义(P<0.001),而牙冠高度未见明显差异。结论:外科手术联合正畸牵引适用于年轻弯曲恒前牙的治疗,有利于牙根发育和唇侧牙槽骨的改建。

Objective To evaluate the efficacy of 30 cases of dilaceration of maxillary central incisors treated by surgical exposure and orthodontic treatment.Methods The children with dilaceration of maxillary central incisors were selected at AnHui Stomatological Hospital from 2019.The affected tooth was used as the observation group and the contralateral tooth of the same name was used as the normal control group,in cases where both central incisors were damaged,the lateral incisors in the same jaw were used as the control group.The post-operative follow-up periods are over 24 months.Evaluated the thickness of labial alveolar bone,the height of tooth crown and the length of the root in the observation and control groups after treatment by CBCT.Results 29 children with 30 dilacerated teeth including 15 boys and 14 girls and 3 cases were lost to follow-up,aged 7-10 with an average age of(7.98±0.63)years.The success rate was 96.3%.Compared to the control group,the results of CBCT showed the thickness of labial alveolar bone and the length of the root were significantly lower in the treated group(P<0.001)Conclusion Dilaceration of maxillary central incisors could be successfully treated by surgical exposure and orthodontic treatment.It benefits the root development and labial alveolar bone reconstruction.
