Er:YAG激光辅助龈下刮治和根面平整术治疗慢性牙周炎伴2型糖尿病的临床疗效

Efficacy of Er:YAG laser assisted subgingival scaling and root planing in the treatment of chronic periodontitis with type 2 diabetes mellitus

目的 评价ER:YAG激光辅助龈下刮治和根面平整术(SRP)对慢性牙周炎伴2型糖尿病患者的牙周临床指标及血清超敏C反应蛋白(hs-CRP)、糖化血红蛋白(glycated hemoglobin,HbA1c)的影响。方法 选取慢性牙周炎伴2型糖尿病患者35例,采用自身对照,根据随机数据表确定一侧半口为实验组,进行SRP及ER:YAG激光治疗,另一侧半口为对照组单纯SRP治疗。治疗前,治疗后1、3个月分别记录菌斑指数(PLI)、牙龈指数(BI)、探诊深度(PD)、临床附着丧失(CAL)等牙周临床指标,治疗前及治疗后3个月检查hs-CRP及HbA1c水平,分别进行统计分析。结果 治疗后1、3个月,实验组BI、PD、CAL明显低于对照组(P<0.05);PLI两组在1、3个月时均无显著差异(P> 0.05)。治疗后3个月hs-CRP及HbA1c水平明显低于治疗前(P<0.05)。结论 使用ER:YAG激光辅助治疗慢性牙周炎伴2型糖尿病的疗效优于单纯牙周基础治疗,可以有效减轻患者牙周炎症,同时也有利于控制血糖水平。

Objective To evaluate the effect of ER:YAG laser(ERL)assisted scaling and root planning(SRP)on periodontal clinical indicators and serum hypersensitivity C-reactive protein(hs-CRP)and glycated hemoglobinglo(HbA1c)in patients with type 2 diabetes and chronic periodontitis.Methods A total of 35 cases with type 2 diabetes and chronic periodontitis were selected.Split-mouth design and one side were determined to be the experimental group according to the random data sheet for SRP and ERL treatment,and the other side was the control group for simple SRP treatment only.Plaque index(PLI),gingival index(BI),probing depth(PD),clinical attachment loss(CAL)and other periodontal clinical indexes were recorded before and 1 and 3 months after treatment.The levels of hs CRP and HbA1c were examined before and 3 months after treatment for statistical analysis.Results At 1 and 3 months after treatment,BI,PD and CAL in the experimental group were significantly lower than those in the control group(P<0.05).There was no significant difference in PLI between the two groups at 1 and 3 months(P>0.05).The levels of hs CRP and HbA1c 3 months after treatment were significantly lower than those before treatment(P<0.05).Conclusions The efficacy of ER:YAG laser assisted treatment for chronic periodontitis with type 2 diabetes is better than that of basic periodontal treatment.It can effectively reduce periodontal inflammation and control blood sugar level.
