摘要
目的 评价浓缩生长因子用于牙龈瘤术后软组织缺损修复的临床疗效。方法 选取2019年10月-2021年5月期间就诊的40例临床诊断为牙龈瘤的患者,随机分2组,实验组采用冠向复位瓣联合浓缩生长因子(concentrated growth factor,CGF)修复,对照组采用单独冠向复位瓣修复。术后1周、1、3、6个月随访,观察记录术后牙龈的角化龈宽度(keratinized gingival width,KGW)、出血指数(bleeding index,BI)、牙周探诊深度(periodontal probing depth,PD)、根面覆盖率(root coverage,RC)、牙龈乳头充盈指数(papilla filled index,PFI)和视觉模拟评分法(visual analogue scale,VAS)。结果 术后3、6个月时两组KGW比较差异均有统计学意义,CGF组在术后1周、3、6个月KGW逐渐增宽。术后3、6个月时两组BI、PD、PFI比较差异均有统计学意义(P <0.05)。术后6个月时两组RC比较差异有统计学意义(P<0.05)。术后1、3、6个月时两组VAS比较差异有统计学意义(P <0.05)。结论 牙龈瘤切术后行CGF+冠向复位瓣修复相较于单纯冠向复位瓣修复,可有效增加角化龈宽度,降低出血指数,增大根面覆盖率及牙龈充盈指数,可作为牙龈瘤术后软组织缺损修复的选择。
Objective To evaluate the clinical effect of concentrated growth factor in the repair of soft tissue defects after epulis surgery.Methods A total of 40 patients with clinically diagnosed epulis were randomly divided into two groups.The experimental group was repaired with coronally advanced flap(CAF)combined with concentrated growth factor(CGF),the control group was repaired with coronally advanced flap.The keratinized gingival width(KGW),bleeding index(BI),periodontal probing depth(PD),root coverage(RC),papilla filled index(PFI)and visual analogue scale(VAS)were recorded.Results There was significant difference in KGW between the two groups 3 and 6 months after operation.KGW gradually widened in CGF group one week and 3 and 6 months after operation.There were significant differences in Bi,PD and PFI between the two groups 3 and 6 months after operation(P<0.05).There was significant difference in RC between the two groups 6 months after operation(P<0.05).VAS was different between the two groups 1,3 and 6 months after operation(P<0.05).Conclusions CGF+CAF after gingivectomy can effectively increase the width of keratinized gingiva,reduce the bleeding index,and increase the root coverage and gingival filling index.It can be used as the choice of soft tissue defect repair after epulis surgery.
