基于阴火理论探析干眼的中医内涵

Discussion on Traditional Chinese Medicine connotation of dry eye based on Yin-Fire theory

脾胃学说为后世医家治疗疾病提供广泛思路,本文将以脾胃学说中的阴火理论为基础,探析干眼的中医内涵。目珠与脾胃在生理和病理上有密切联系,脾胃运化的水谷精微以及转化成的血起到濡养作用,有助于维持眼珠的正常功能;脾失健运,脾胃气虚导致阴火内盛、湿热内生、肝木亢旺、脾不统血将引起眼珠功能失常。干眼分为“热、郁、虚”3个阶段,干眼作为慢性低度炎症是由于脾胃气虚,气机升降失常,阴火内盛,灼伤阴液所致,日久累及他脏,出现严重并发症。从慢性低度炎症入手,认为阴火理论与自噬功能异常引起的炎症反应相关,慢性低度炎症引起了干眼的发病。阴火理论进一步完善了脾胃学说在干眼诊治方面的应用。

The theory of spleen and stomach provides a broad idea for Traditional Chinese Medicine(TCM)doctors of later generations to treat diseases.This paper will discuss on the TCM connotation of dry eye based on the theory of Yin-Fire in the theory of spleen and stomach.The eyes are closely related to the spleen and stomach in physiology and pathology.The nourishing effect of the food essence and water as well as the transformed blood of the spleen and stomach helps to maintain the normal function of the eyes.Once the spleen loses its healthy function,and Qi deficiency of the spleen and stomach leads to the internal excess of Yin-Fire,the internal generation of damp and heat,the hyperactivity of the liver,and the diability of the spleen in governing blood will cause the abnormal function of the eyes.Dry eye can be divided into three stages:"heat,depression and deficiency".As a chronic low-grade inflammation,it is caused by Qi deficiency of the spleen and stomach,abnormal ascending and descending of Qi activity,excessive Yin-Fire which burns Yin fluid.Over time,other organs can be affected,causing serious complications.From the perspective of chronic low-grade inflammation,it is believed that Yin-Fire theory is related to the inflammation caused by abnormal autophagy function,and this kind of chronic low-grade inflammation causes the onset of dry eye.Yin-Fire theory further improves the application of spleen and stomach theory in the diagnosis and treatment of dry eye.
